目的比较全数字化乳腺摄影(full-fielddigitalmammography,FFDM)与磁共振成像(MRI)对乳腺癌的诊断价值。方法选取2003年4月-2006年4月期间行FFDM并怀疑有乳腺病变的100例患者作为研究对象,其中良性病变46例,恶性病变54例,均经病理证实。全部病例在一周之内同时行MRI检查。结果良性病变46例中,临床触及不到肿块6例,其病理结果:纤维腺瘤5例、腺病1例;临床触及肿块40例,其病理结果:纤维腺瘤25例、乳腺囊性增生症4例、乳腺腺病3例、乳腺结核1例、乳腺囊肿7例。恶性病变54例中,临床触及不到肿块8例,病理结果:癌前病变1例、导管内癌3例、浸润导管癌3例、黏液腺癌1例;临床触及包块46例,病理结果:浸润性导管癌44例,黏液腺癌2例。本组研究显示FFDM对乳腺癌诊断的敏感性、特异性和准确性分别为92.59%、91.30%、92%;MRI对乳腺癌诊断的敏感性、特异性和准确性分别为85.19%、86.96%、87%。结论全数字化乳腺摄影对乳腺癌的诊断价值优于MRI。 Objective To compare the diagnostic value of full-field digital mammography (FFDM) and magnetic resonance imaging (MRI) in breast cancer. Methods A total of 100 patients with FFDM suspected breast lesions from April 2003 to April 2006 were selected as the study subjects, 46 of them were benign lesions and 54 were malignant lesions. All were pathologically confirmed. All patients underwent MRI at the same time within one week. Results In 46 cases of benign lesions, no clinical lump was found in 6 cases. The pathological results included 5 cases of fibroadenoma and 1 case of adenopathy; 40 cases were clinically palpated. The pathological result was fibroid adenoma in 25 cases and cystic hyperplasia of the breast. 4 cases, 3 cases of breast adenosis, 1 case of breast tuberculosis, and 7 cases of breast cyst. In 54 cases of malignant lesions, no clinical lump was found in 8 cases. Pathological findings included 1 precancerous lesion, 3 intraductal carcinomas, 3 invasive ductal carcinomas, and 1 mucinous adenocarcinoma; 46 palpable masses were involved in pathological findings. : 44 invasive ductal carcinomas and 2 mucinous adenocarcinomas. This group of studies showed that the sensitivity, specificity, and accuracy of FFDM in the diagnosis of breast cancer were 92.59%, 91.30%, and 92%, respectively; the sensitivity, specificity, and accuracy of MRI in the diagnosis of breast cancer were 85.19%, 86.96%, respectively. 87%. Conclusions All-digital mammography is superior to MRI in the diagnosis of breast cancer.
