目的探讨64层螺旋CT血管成像技术在下肢动脉硬化性闭塞症(arteriosclerotic occlusive disease,ASO)诊断中的技术方法及应用价值。方法对59例临床疑有ASO的患者行64层螺旋CT血管成像检查,将数据在AW4.1工作站行容积再现(volume rendering,VR)、最大密度投影(maximum intensity projection,MIP)及其反转像(Inverse-MIP)、曲面重建(curved planar reformation,CPR)及高级血管分析(advanced vessel analysis,AVA)等多种后处理并结合原始图像进行综合分析。结果59例的血管重建图像显示良好,正常4例,55例存在不同程度的阻塞,呈节段性或跳跃性,闭塞动脉周围较多代偿侧支血管。结论64层螺旋CT血管成像检查可清楚地观察下肢动脉的血管形态及管壁的钙化斑块,了解下肢动脉狭窄程度及闭塞情况,对下肢动脉硬化性闭塞性病变的术前评价、诊断和筛选有较大的帮助。
Objective To explore the technique and application of 64-slice spiral CT angiography in the diagnosis of arteriosclerosis occlusive disease (ASO) in lower extremity. Methods Sixty-nine patients with suspected ASO were examined by 64-slice spiral CT angiography, and the data were processed on an AW4.1 workstation with volume rendering (VR), maximum intensity projection (MIP) Such as Inverse-MIP, curved planar reformation (CPR) and advanced vessel analysis (AVA), combined with the original images. Results In 59 cases, the images of revascularization showed good and normal in 4 cases. There were 55 cases with different degrees of occlusion, segmental or jumping, and more compensatory collateral vessels around the occluded artery. Conclusion 64-slice spiral CT angiography can clearly observe the vascular morphology of lower extremity arteries and calcified plaque on the wall to understand the extent and occlusion of lower extremity arterial stenosis and evaluate the preoperative evaluation, diagnosis and screening of lower extremity arteriosclerotic occlusive disease Have greater help.
