目的研究分析血浆C反应蛋白(CRP)和同型半胱氨酸水平(Hcy)与2型糖尿病周围神经病变的相关性。方法在本院住院治疗的糖尿病患者中选取166例作为研究对象,所有研究对象均于2016年1月~2016年12月期间入院,将患者按神经自主症状和震动感觉阈值检查、肌电图检查分为单纯糖尿病组(SDM组)和2型糖尿病周围神经病变组(DPN组),分别为70例和96例,对所有研究对象的CRP、Hcy、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、空腹血糖(FBG)、甘油三酯(TG)、糖化血红蛋白(Hb AlC)水平进行检测,并进行比较分析。结果 2型糖尿病周围神经病变组与单纯糖尿病组的Hb AlC、LDL-C水平存在较大差别,两组数据比较差异有统计学意义(P<0.05);2型糖尿病周围神经病变组与单纯糖尿病组的HDL-C、FBG、TC、TG水平差异较小,两组数据比较差异无统计学意义;2型糖尿病周围神经病变组与单纯糖尿病组的CRP、Hcy水平差异显著,有统计学意义(P<0.05);Hb AlC、LDL-C、CRP、Hcy是糖尿病周围神经病变发生的独立危险因素。结论血浆C反应蛋白(CRP)和同型半胱氨酸水平(Hcy)与2型糖尿病周围神经病变有着紧密关联,可通过测定CRP、Hcy来对2型糖尿病周围神经病变进行筛查。
Objective To study the relationship between plasma C-reactive protein (CRP) and homocysteine (Hcy) and type 2 diabetic peripheral neuropathy. Methods A total of 166 diabetic patients admitted to our hospital were enrolled in the study. All patients were admitted to our hospital from January 2016 to December 2016. The patients underwent autonomic symptoms and shock sensation threshold tests. Electromyography (SDM group) and type 2 diabetic peripheral neuropathy group (DPN group) were divided into two groups: 70 cases and 96 cases. The CRP, Hcy, high density lipoprotein cholesterol (HDL-C) LDL-C, TC, FBG, TG and Hb AlC were measured and compared. Results The levels of Hb AlC, LDL-C in type 2 diabetic peripheral neuropathy group were significantly different from those in simple diabetic group (P <0.05). There were significant differences between the two groups (P <0.05) There was no significant difference in HDL-C, FBG, TC and TG levels between the two groups, but no significant difference was found between the two groups. There was significant difference in CRP and Hcy between type 2 diabetic peripheral neuropathy group and diabetic group P <0.05). Hb AlC, LDL-C, CRP and Hcy were independent risk factors of diabetic peripheral neuropathy. Conclusions Plasma C-reactive protein (CRP) and homocysteine levels (Hcy) are closely related to peripheral neuropathy in type 2 diabetic patients. Type 2 diabetic peripheral neuropathy can be screened by measuring CRP and Hcy.
