目的比较东丽区无瑕街社区卫生服务中心宫颈癌免费筛查时使用液基细胞学检查技术(T C T)与宫颈刮片技术在最终检查结果的阳性检出率和特异度方面的比较差异。方法回顾2 0 1 2—2 0 1 3年免费宫颈癌筛查例数7 4 8 7例,其中,使用宫颈刮片技术筛查4 2 5 4例,使用T C T技术筛查3 2 3 3例,以最终的宫颈组织病理结果作为两种检查技术的检验手段进行比较。结果两组检查技术筛查人群在年龄结构方面的差异无统计学意义(P>0.0 5),具有可比性。宫颈刮片结果检出1 9例异常结果,追访经进一步诊治病理学诊断C INⅠ1例,C INⅡ0例,C INⅢ0例,子宫颈浸润癌1例。T C T共查出3 1例异常结果,经上级医院诊治最终病理学诊断C INI 3例,C INⅡ1例,C INⅢ0例,子宫颈原位癌3例,子宫颈浸润癌1例。两组筛查技术的特异度进行比较,差异有统计学意义(P<0.0 5),T C T检查技术的特异度高于宫颈刮片检查技术。结论 T C T技术是2 0世纪9 0年代的新兴技术,在大规模宫颈癌普查中宫颈上皮内瘤变和子宫颈癌的检出率比宫颈刮片高,假阳性率比宫颈刮片低,假阴性率比宫颈刮片也低,尤其对宫颈上皮内瘤变(C IN)的敏感性较高,宫颈癌起源于宫颈上皮内瘤变,宫颈上皮内瘤变的早发现避免了宫颈癌的发生。
Objective To compare the difference of the positive detection rate and specificity between liquid-based cytology (T C T) and cervical smear technique in the free screening of cervical cancer in the flawless street community health service center of Dongli District. Methods The number of free cervical cancer screening cases in 2001-2003 was retrospectively analyzed. Among them, 42 254 cases were screened by cervical smear technique and 3323 cases by TCT technique , The final results of cervical tissue pathology as a test of two inspection techniques for comparison. Results There was no significant difference in age structure between two groups of screening techniques (P> 0.05), which was comparable. Cervical smear results found abnormal results of 19 cases, followed by further diagnosis and treatment pathological diagnosis of C IN Ⅰ cases, C IN Ⅱ 0 cases, C IN Ⅲ 0 cases, 1 case of cervical invasive carcinoma. T c T were detected in 31 cases of abnormal results, diagnosis and treatment by the higher hospital final pathology C INI 3 cases, C IN Ⅱ 1 cases, C IN Ⅲ 0 cases, 3 cases of cervical carcinoma in situ, 1 case of cervical cancer. The specificity of the two screening techniques was compared, the difference was statistically significant (P <0.0 5), T C T examination of specificity is higher than the cervical smear examination. Conclusion TCT technology is a new technology in the 1990s. In the large-scale cervical cancer screening, the detection rate of cervical intraepithelial neoplasia and cervical cancer is higher than that of cervical smear, the false positive rate is lower than that of cervical smear, false negative Cervical cancer is also lower than the cervical smear, especially for cervical intraepithelial neoplasia (CIN) higher sensitivity, cervical cancer originated in cervical intraepithelial neoplasia, early detection of cervical intraepithelial neoplasia to avoid the occurrence of cervical cancer.
