目的观察动态血糖监测系统(CGMS)对老年2型糖尿病(T2DM)患者血糖控制水平及健康教育效果的中期影响。方法老年T2DM患者88例,随机分为CGMS组和对照组(多点血糖监测组)各44例,CGMS组应用CGMS监测血糖72h,之后采用快速血糖仪常规测量指尖多点血糖7d,对照组常规测量指尖多点血糖10d。结果CGMS组、对照组空腹血糖(FPG)〔(5.89±0.97)vs(6.76±0.18)mmol/L〕、早餐后2h血糖〔(8.27±0.29)vs(9.36±0.41)mmol/L〕、晚餐后2h血糖〔(8.74±1.75)vs(9.98±2.56)mmol/L〕、睡前血糖〔(7.65±1.26)vs(9.04±2.07)mmol/L〕、凌晨3点血糖〔(6.20±0.71)vs(8.90±0.57)mmol/L〕,血糖达标时间〔(4.62±1.32)vs(7.51±1.28)d〕,低血糖发生次数〔(5.20±1.21)vs(8.52±2.34)次〕,平均住院天数〔(19.25±20.34)vs(25.66±21.60)d〕,3个月后糖化血红蛋白〔(6.41±2.13)vs(8.02±2.04)%〕,糖尿病知识问卷得分〔(22.13±1.94)vs(16.90±2.93)〕均有显著差异(P<0.05,P<0.01)。结论CGMS能更准确地提供血糖波动情况,对老年糖尿病患者中期的血糖控制和健康教育效果均具有重要的价值。
Objective To observe the medium-term effect of dynamic glucose monitoring system (CGMS) on blood glucose control and health education in elderly patients with type 2 diabetes mellitus (T2DM). Methods 88 elderly patients with T2DM were randomly divided into CGMS group and control group (multi-point blood glucose monitoring group), 44 cases each. The CGMS group was monitored by CGMS for 72 hours, then the fast glucose meter was used to measure the finger tip blood glucose for 7 days, Routine measurement of blood glucose more than 10d. Results The levels of fasting plasma glucose (FPG) (5.89 ± 0.97) vs (6.76 ± 0.18) mmol / L in CGMS group and control group were significantly higher than those in control group (8.27 ± 0.29 vs 9.36 ± 0.41 mmol / L, The blood glucose at the second hour was (8.74 ± 1.75) vs (9.98 ± 2.56) mmol / L, before bedtime blood glucose was (7.65 ± 1.26) vs (9.04 ± 2.07) mmol / L, (8.90 ± 0.57) mmol / L〕, blood glucose compliance time 〔(4.62 ± 1.32) vs (7.51 ± 1.28〕 d〕, incidence of hypoglycemia 〔(5.20 ± 1.21) vs (8.52 ± 2.34〕〕〕, (19.25 ± 20.34) vs (25.66 ± 21.60) d〕, and the scores of glycosylated hemoglobin after 3 months 〔(6.41 ± 2.13) vs (8.02 ± 2.04)%〕, diabetes knowledge questionnaire score 〔(22.13 ± 1.94〕 vs (16.90 ± 2.93)] were significantly different (P <0.05, P <0.01). Conclusions CGMS can provide more accurate blood sugar fluctuation, which is of great value to the elderly patients with diabetes in the mid-term blood glucose control and health education.
