目的:分析脑脊液培养阳性的婴幼儿化脓性脑膜炎病原菌分布及药敏情况,为临床诊治提供参考。方法对百色市妇幼保健院和百色市凌云县人民医院2010年1月~2014年8月31日检出的脑脊液病原菌培养阳性的患儿进行病原菌及药敏结果进行回顾性分析。结果脑脊液细菌培养阳性共32例,分离病原菌32株,其中革兰阳性菌27株,占84.38%,均以凝固酶阴性葡萄球菌为主,占71.87%,前4位病原菌分别是表皮葡萄球菌、人葡萄球菌、耳葡萄球菌和溶血葡萄球菌;革兰阴性菌占15.63%。前四位病原菌对青霉素、氨苄西林、红霉素高度耐药,对前四位病原菌敏感药物是万古霉素、亚胺培南、环丙沙星、丁胺卡那及第三代头孢菌素,亚胺培南及万古霉素高度敏感,未发现耐药。结论本研究发现表皮葡萄球菌、人葡萄球菌、耳葡萄球菌和溶血葡萄球菌为婴幼儿化脓性脑膜常见病原菌;万古霉素、亚胺培南及第三代头孢菌素可作为儿童化脓性脑膜炎首选抗菌药物。,Objective To investigate the distribution and drug resistance of pathogenic bacteria in infant purulent meningitis with positive cerebral spinal fluid culture results,and provide evidence for clinical therapy.Methods Bacteria sensitive date of the children with purulent meningitis in Baise Matal and Child Health-Care Hospital and Lingyun County People’s Hospital of Baise from January 2010 and August 31,2014 were retrospectively analyzed. Results From the 32 cerebrospinal fluid specimens,32 bacterial strains were isolated that including 27 (84.38%) gram-positive strains.The strains are mainly coagulase negative staphylococcus (71.87%),and the leading four pathogenic bacteria were epidermis staphylococcus aureus、staphylococcus aureus、ear staphylococcus and hemolysis staphylococcus,which are highly resistant to penicillin、ampicillin、erythromycin and sensitive to pathogenic bacteria drug vancomycin、imine south、ciprofloxacin、amikacin.Conclusion The common pathogenic bacteria of purulent meningitis in infant is epidermis staphylococcus aureus,staphylococcus aureus,ear staphylococcus and hemolytic staphylococci. Vancomycin,imine culture south of the 3 (superscript rd) generation cephalosporin fester sex meningitis can be used as first choice of children suppurative meningitis.
