目的研究侧卧位行经脐单孔腹腔镜技术(E-NOTES)的疗效及不良反应。方法于2010年5月至2012年1月,对26例上尿路疾病患者采用侧卧位行E-NOTES治疗。另选26例尿路疾病患者行传统切开手术处理作为对照,对比两组患者平均手术时间、术中出血、术后引流量、术后胃肠功能恢复时间以及术后平均住院时间。结果所有患者手术均顺利进行,术中视野良好。侧卧位E-NOTES组患者手术时间、术中出血量、术后胃肠功能恢复时间、术后住院时间显著低于开放手术组,差异有统计学意义(P<0.05)。结论侧卧位行E-NOTES治疗上尿路疾病安全可行,术中肠管的干扰明显减少,相应手术部位得以充分暴露,有利手术的开展,可减少工作强度,值得临床选用。
Objective To study the curative effect and side effects of lateral transthoracic esophagectomy via E-NOTES. Methods From May 2010 to January 2012, 26 cases of upper urinary tract diseases were treated with E-NOTES. Another 26 cases of urinary tract disease patients underwent traditional incision surgery as a control. The mean operative time, intraoperative bleeding, postoperative drainage volume, postoperative gastrointestinal function recovery time and postoperative average hospital stay were compared between the two groups. Results All the patients underwent surgery with good visual field. The operation time, intraoperative blood loss, postoperative gastrointestinal function recovery time and postoperative hospital stay in E-NOTES group were significantly lower than those in open surgery group (P <0.05). Conclusion E-NOTES treatment of upper urinary tract disease in the lateral position is safe and feasible. The interference of intestinal tube in operation is obviously reduced. The corresponding surgical site is fully exposed. It is advantageous for the operation to be performed, which can reduce the work intensity and deserve clinical application.
