Objective To compare the MRI findings of multiple sclerosis (MS) and optic neuromyelitis (NMO) optic neuropathy and to explore its value in the diagnosis and differential diagnosis. Methods Twenty patients with MS and 11 patients with NMO accompanied with optic nerve damage underwent optic nerve MRI with 3.0 T magnetic resonance imaging. The distribution of optic nerve lesions in MS and NMO was compared. The distribution of optic nerve lesions and the length of optic nerve lesions were compared . Results The incidence of peripheral optic nerve lesions in MS group was significantly higher than that in NMO group (P <0.05). The incidence of NMO intracranial and intracranial lesions was higher than that in MS group (P <0.05), and the difference was statistically significant (P <0.05). The length of optic neuropathy in NMO group was longer than that in MS group (P <0.05). Conclusion There is a significant difference between MS and NMO in optic neuropathy. Optic nerve MRI has great clinical value in the diagnosis and differential diagnosis of NMO and MS optic neuropathy.
