目的:探讨Survivin和CyclinD2在乳腺导管上皮内瘤变(DIN)中的表达情况,寻求有助于乳腺导管上皮内瘤变诊断的分子标记。方法:通过免疫组化SP法检测20例普通型导管增生(UDH)、15例DIN1A、30例DIN1B、21例DIN1C-3组织中的Survivin和CyclinD2表达情况,并以正常乳腺组织作为对照。结果:Survivin在UDH/DIN1A、DIN1B和DIN1C-3组中的阳性率分别为0、36.6%和81.0%。DIN1B和DIN1C-3两组比较差异有统计学意义(P<0.001)。CyclinD2在UDH、DIN1A、DIN1B和DIN1C-3组中的表达率分别为10%、13.3%、41.9%和76.2%,UDH、DIN1A与DIN1B和DIN1C-3比较差异有统计学意义(P<0.001),DIN1A与DIN1C-3两组比较差异亦有统计学意义(P<0.001),UDH与DIN1A比较差异无统计学意义(P>0.05)。结论:从正常乳腺组织到UDH、DIN1A、DIN1B及DIN1C-3的恶性转化过程中伴有Survivin和CyclinD2表达的逐渐增高,其过度表达可能是某些乳腺癌发生过程中的早期事件,Survivin和CyclinD2可作为乳腺UDH、DIN1A与DIN1B和DIN1C-3鉴别诊断的标记物。
OBJECTIVE: To investigate the expression of Survivin and CyclinD2 in ductal intraepithelial neoplasia (DIN), and to find molecular markers that contribute to the diagnosis of ductal intraepithelial neoplasia. Methods: The expression of Survivin and CyclinD2 in 20 cases of common ductal hyperplasia (UDH), 15 cases of DIN1A, 30 cases of DIN1B and 21 cases of DIN1C-3 tissues were detected by immunohistochemical SP method. Normal breast tissues were used as control. Results: The positive rates of Survivin in UDH / DIN1A, DIN1B and DIN1C-3 groups were 0, 36.6% and 81.0% respectively. There was significant difference between DIN1B and DIN1C-3 (P <0.001). The expression rates of CyclinD2 in UDH, DIN1A, DIN1B and DIN1C-3 groups were 10%, 13.3%, 41.9% and 76.2% respectively. There was significant difference between UDH, DIN1A and DIN1B and DIN1C-3 (P <0.001) The difference between DIN1A and DIN1C-3 was also statistically significant (P <0.001). There was no significant difference between UDH and DIN1A (P> 0.05). CONCLUSIONS: The expression of Survivin and CyclinD2 is gradually increased during the malignant transformation from normal breast tissues to UDH, DIN1A, DIN1B and DIN1C-3. The overexpression of Survivin and CyclinD2 may be early events in some breast carcinomas. Survivin and CyclinD2 Can be used as breast UDH, DIN1A and DIN1B and DIN1C-3 differential diagnosis of markers.
