目的探讨肺癌患者化疗前后生活质量情况及影响因素。方法选择2012年1月—2014年12月在浙江省金华市广福医院呼吸科被诊断为晚期肺癌的患者86例作为研究对象,收集患者的病史资料,进行正规化疗治疗,在化疗前及化疗4个周期后进行EORTC QLQ-30问卷调查。观察肺癌患者化疗前后生活质量评分变化、肺癌患者的健康平均评分和影响因素的关系、肺癌患者的EORTC QLQ-30其它条目评分和影响因素的关系。结果晚期肺癌患者化疗4个周期后的躯体评分、情绪评分、社会评分、呼吸困难评分、总健康状况评分均明显低于化疗前(P<0.05),晚期肺癌患者化疗4个周期后的恶心呕吐评分、失眠评分、食欲减退评分、便秘腹泻评分均明显高于化疗前,差异有统计学意义(P<0.05),晚期肺癌患者的健康平均评分和患者的年龄、性别、病理类型、文化程度以及基础疾病之间差异无统计学意义(P>0.05)。女性肺癌患者的食欲下降评分高于男性患者(P<0.05),60岁以上患者的食欲下降评分、便秘评分和恶心评分高于60以下患者(P<0.05),文化程度比较低的患者失眠评分高于文化程度比较高的患者(P<0.05),有基础疾病患者的疲倦评分高于无基础疾病患者(P<0.05),差异均有统计学意义。结论晚期肺癌患者化疗后躯体评分、情绪评分、社会评分、呼吸困难评分、总健康状况评分降低,恶心呕吐评分、失眠评分、食欲减退评分、便秘腹泻评分升高,晚期肺癌患者的生活质量情况和患者的性别、年龄、文化程度和是否合并基础疾病有关。
Objective To investigate the quality of life and influencing factors of patients with lung cancer before and after chemotherapy. Methods From January 2012 to December 2014, 86 patients with advanced lung cancer who were diagnosed as advanced lung cancer from Guangfu Hospital of Jinhua City, Zhejiang Province from December 2012 to December 2014 were enrolled in this study. Their medical history data were collected for formal chemotherapy. Before chemotherapy and chemotherapy EORTC QLQ-30 questionnaire was conducted after 4 cycles. To observe the change of quality of life score of patients with lung cancer before and after chemotherapy, the average score of healthy patients with lung cancer and the influencing factors, lung cancer patients with EORTC QLQ-30 scores of other items and the relationship between influencing factors. Results The body score, emotion score, social score, dyspnea score and total health score of patients with advanced lung cancer after 4 cycles of chemotherapy were significantly lower than those before chemotherapy (P <0.05). The patients with advanced lung cancer had nausea and vomiting after 4 cycles of chemotherapy Score, insomnia score, loss of appetite score, constipation and diarrhea scores were significantly higher than before chemotherapy, the difference was statistically significant (P <0.05), the average score of patients with advanced lung cancer and the patient’s age, gender, pathological type, educational level and There was no significant difference between the underlying diseases (P> 0.05). The decline in appetite of female patients with lung cancer was higher than that of male patients (P <0.05). The appetite descending score, constipation score and nausea score were higher than 60 in patients over 60 years old (P <0.05) (P <0.05). The scores of fatigue in patients with underlying diseases were higher than those in patients without underlying diseases (P <0.05), and the differences were statistically significant.
