目的探索医院门诊药房处方调剂出错原因,就出错原因进行对策分析。方法 85例门诊药房出现的处方调剂差错记录为研究对象,回顾性分析造成差错的原因并提出具有针对性的对策。结果药房处方调剂的差错主要由3方面原因造成:1临床医师方面:a.临床医师不规范的书写;b.临床医师各种原因导致的处方内容错误;c.临床医师未对相同药物批次、型号等进行批注。2门诊药师方面:a.门诊药师缺乏对临床相关专业知识的掌握;b.缺乏责任心,未按照处方严格执行;c.临床药师在分发药品时没有对特殊药物进行标注、交代易造成混淆。3环境方面:a.药品不合理的摆放造成误拿、错拿;b.药房环境过于狭小,在高峰期易造成拥堵易出差错。4其他原因:药品本身具有质量问题等。结论患者的疗效与药物有着直接的关系,而门诊药房则是医患之间一条重要的纽带,在医患关系紧张的今天,门诊药房处方的差错则会加剧医患的矛盾,因此临床医师要规范书写,门诊药师更要储备充足的临床知识,培养良好的责任心,医院尽可能改善药房的客观条件等,这样医院才会有更好发展,医患关系得以缓解。
Objective To explore the reasons for the errors in prescription dispensation of outpatient pharmacy and analyze the causes of the errors. Methods Eighty - five outpatient pharmacies were asked to record the errors of prescription adjustment as the research object, the causes of the errors were retrospectively analyzed and the corresponding countermeasures were put forward. Results Pharmacy prescription transfer error mainly caused by three aspects: 1 clinicians: a. Clinicians do not regulate the writing; b. Clinicians cause a variety of errors in the contents of the prescription; c. Clinicians not the same drug batch , Model number, etc. are annotated. 2 outpatient pharmacists: a. Outpatient pharmacists lack of clinical expertise; b. Lack of sense of responsibility, not strictly enforced in accordance with prescriptions; c. Clinical pharmacists in the distribution of drugs did not label special drugs, confiscation can easily lead to confusion. 3 environmental aspects: a. Improper placement of medicines caused by mistake, wrong to take; b. Pharmacy environment is too small, easily lead to congestion at peak congestion prone to error. 4 other reasons: the drug itself has quality problems. Conclusions The curative effect of the patients is directly related to the drugs. However, the outpatient pharmacy is an important link between doctors and patients. In the tension between doctors and patients, the prescription of outpatient pharmacies can aggravate the contradiction between doctors and patients. Therefore, clinicians should To standardize writing, outpatient pharmacists should reserve sufficient clinical knowledge, cultivate a good sense of responsibility, and hospitals should try their best to improve the objective conditions of pharmacies so that the hospital will have better development and the relationship between doctors and patients will be alleviated.
