目的 :探讨Ⅲ级产前超声检查在降低新生儿出生畸形中的临床应用价值。方法 :选取2012年1月~2015年6月在本院超声科接受产前Ⅲ级超声检查的2570例妊娠妇女作为研究组、同期仅接受了Ⅰ级、Ⅱ级超声检查的4000例妊娠妇女作为对照组,以分娩结局、引产后尸检结局作为金标准,对比两组阳性检出率的、产前诊断胎儿畸形中的价值。结果 :研究组采用产前Ⅲ级超声进行检查,结果正确检出23例缺陷胎儿,误诊5例、漏诊3例,产前Ⅲ级超声诊断缺陷胎儿的灵敏度为88.46%、特异度为99.81%、漏诊率为11.54%、误诊率为0.19%;研究组采用产前Ⅰ级或Ⅱ级产前超声进行检查,结果正确检出29例缺陷胎儿,误诊15例、漏诊12例,产前Ⅰ级、Ⅱ级超声诊断缺陷胎儿的灵敏度为70.73%、特异度为99.62%、漏诊率为29.27%、误诊率为0.38%;产前Ⅲ级超声诊断缺陷胎儿的灵敏度显著的高于产前Ⅰ级或Ⅱ级超声检查。结论 :Ⅲ级产前超声检查早期发现缺陷胎儿的能力强于Ⅰ级或Ⅱ级超声,有利于减少出生缺陷。
Objective: To investigate the clinical value of grade Ⅲ prenatal ultrasound in reducing birth deformity. Methods: From January 2012 to June 2015, 2570 pregnant women undergoing antenatal stage Ⅲ ultrasonography in our department of ultrasound were enrolled as the study group. In the same period, 4,000 pregnant women who underwent grade Ⅰ and Ⅱ ultrasonography In the control group, the final result of delivery and autopsy outcome were regarded as the gold standard. The positive rate of prenatal diagnosis of fetus deformity was compared between the two groups. Results: The study group was diagnosed with prenatal Ⅲ ultrasonography. The results showed that 23 fetuses with defective fetus were correctly detected, 5 were misdiagnosed and 3 missed. The sensitivity, specificity and accuracy of prenatal Ⅲ ultrasonography were 88.46% and 99.81%, respectively. The rate of misdiagnosis was 11.54% and the rate of misdiagnosis was 0.19%. The study group used prenatal grade Ⅰ or Ⅱ prenatal ultrasound to detect 29 cases of defective fetus, misdiagnosed 15 cases, missed diagnosis of 12 cases, prenatal grade Ⅰ, The sensitivity of grade Ⅱ ultrasound fetal defect was 70.73%, the specificity was 99.62%, the rate of missed diagnosis was 29.27%, the misdiagnosis rate was 0.38%. The prenatal sensitivity of fetus with grade Ⅲ ultrasonography was significantly higher than that of prenatal grade Ⅰ or Ⅱ Ultrasound examination. Conclusion: The grade Ⅲ prenatal ultrasound can detect the defective fetus more early than the grade Ⅰ or Ⅱ ultrasound, which helps to reduce the birth defects.
