目的:旨在探索合理的手术入路,进一步提高延髓腹外侧区病变的手术疗效。方法:取枕下极外侧经髁入路,显微手术治疗7例延髓腹外侧区肿瘤。结果:肿瘤全切除5例,次全切除2例,无手术死亡。结论:延髓腹外侧区结构重要,解剖复杂,常规手术入路难以到达,枕下极外侧经髁入路,其切口和骨窗更靠外侧,不必牵拉脑干和小脑,术中可经颅神经间隙,脊髓与颅底和椎管的前壁,从而扩大视野和操作空间,更适于延髓腹外侧区肿瘤的显微手术
Objective: To explore a reasonable surgical approach to further improve the surgical outcome of rostral ventrolateral lesions. Methods: 7 cases of rostral ventrolateral medulla tumors were treated by microsurgery. Results: Total tumor resection was performed in 5 cases and subtotal resection in 2 cases. No operative death occurred. Conclusion: The rostral ventrolateral medulla has an important structure and complex anatomy. It is difficult to reach with conventional surgical approach. The lateral pole of the occipital pole is transtibial. The incision and the bone window are more lateral, and it is not necessary to pull the brain stem and cerebellum. The nerve gap, the anterior wall of the spinal cord and the skull base and the spinal canal, thereby expanding the visual field and operating space, and is more suitable for the microsurgery of the rostral ventrolateral medulla
