目的 研究术中植入放射性1 2 5碘粒子对外科吻合口的影响。 方法 1 2只健康杂种犬 ,作小肠横形切断并端端吻合。其中 8只实验犬在吻合口两侧植入1 2 5碘粒子 ,总剂量 1 1 6GY ,另 4只犬作对照。分别在术后 7、1 4d各取实验犬标本 4只及对照犬标本 2只 ,作大体标本观察 ,吻合口组织羟脯氨酸定量测定和病理组织学检查。 结果 所有实验犬吻合口愈合良好。吻合口组织羟脯氨酸定量 :实验犬为 0 578± 0 0 2 0 μg/mg蛋白质 ,对照组为 0 631± 0 0 1 2 μg/mg蛋白质 (P >0 0 5)。在吻合口愈合期内 ,吻合口组织学改变与对照组差异无显著性意义。临床 2 9例吻合口及支气管残端 ,2 8例愈合良好 ,1例颈部食管胃吻合口瘘。 结论 术中植入1 2 5碘粒子对外科吻合口无不良影响 ,是安全可行的
Objective To study the effect of intraoperative implantation of radioactive 1 25 iodine particles on surgical anastomoses. Methods 1 2 healthy mongrel dogs were used to make the small intestine transversely cut and end-to-end anastomosis. Among them, 8 experimental dogs were implanted with 1 2 5 iodine particles on both sides of the anastomotic stoma, and the total dose was 11 6GY. The other 4 dogs were used as controls. Seven experimental dogs and two control dog specimens were taken on postoperative day 7 and day 14 respectively for gross specimen observation. An anastomotic tissue was used for quantitative determination of hydroxyproline and histopathological examination. Results The anastomoses of all experimental dogs healed well. Quantitation of hydroxyproline in the anastomosis tissue: 0 578 ± 0 0 2 0 μg/mg protein in experimental dogs and 0 631 ± 0 0 1 2 μg/mg protein in controls (P > 0 0 5). During the healing period of anastomosis, there was no significant difference in histological changes between the anastomosis and the control group. Twenty-nine patients had anastomoses and bronchial stumps, 28 had good healing, and 1 had neck esophagogastric anastomotic leakage. Conclusion Intraoperative implantation of 125 Iodine particles has no adverse effects on surgical anastomosis and is safe and feasible.
