患儿,女,2 1/2岁,因注射流脑菌苗半小时后面部、眼睑、双耳红肿,声音嘶哑就诊。此为第三次,前两次无不良反应。皮下注射流脑菌苗0.5ml(成都生物制品厂产,批号:900908)30分钟后腹痛,有排便感,喷嚏,面部、眼睑、双耳红肿,声音嘶哑,1小时后红肿加重,双眼不能睁开。体检:烦躁不安,哭声嘶哑,面部、眼睑、双耳红肿,不能睁眼,脉搏有力,心律齐,呼吸音粗糙,腹软、无压痛。立即给予氢化考的松25mg、维生素C0.5g、10％GS100ml静滴,维丁胶性钙1ml肌注,口服扑尔敏1.3mg,维生素C0.1g,3次/日,连服2天。输液30分钟后患儿安静入睡,8小时后全身皮肤出现散在粟粒大小红色丘疹,压后退色,瘙痒。24小时后红肿消退,2天后皮疹消退。本例提示,在预防注射时,应仔细询问变态反应史,常规做好过敏
