目的探讨超声在少见桡神经损伤中的定位诊断价值。方法对7例上臂桡神经缩窄和5例骨间后神经损伤的患者进行超声检查,分析其超声声像图特征,并与临床外科手术进行对照。结果超声可清晰显示上肢桡神经束的走行及内部结构。12例患者中7例超声定位病变于上臂桡神经束,5例病变定位于前臂的骨间后神经,声像图表现为神经走行粗细不均,局部缩窄变细、两端神经束增粗,内部条索束膜结构消失,回声减低,外膜回声增强。超声定位和损伤处形态评估与术中发现一致。结论术前超声检查可明确上臂桡神经卡压及骨间后神经损伤,确定缩窄病灶位置及神经形态变化,对指导手术方案制定及评估预后有重要价值。
Objective To investigate the diagnostic value of ultrasound in the diagnosis of rare radial nerve injury. Methods Seven patients with upper radial nerve narrowing and five patients with posterior interosseous nerve injury underwent ultrasonography. The echocardiographic features were analyzed and compared with clinical surgery. Results Ultrasound can clearly show the upper extremity radial nerve bundle walking and internal structure. Among the 12 patients, 7 cases were located in the radial nerve bundles of the upper arm and 5 cases were located in the posterior interosseous nerve. Ultrasound images showed uneven thickness of the nerve, narrowing locally, thickening of the nerve bundles at both ends , The internal structure of the fascicle disappears, the echo is reduced, the outer membrane echo enhancement. Ultrasound localization and damage at the morphological evaluation and intraoperative findings. Conclusion Preoperative ultrasonography can confirm the compression of upper radial nerve and the injury of posterior interosseous nerve and determine the position of narrowing lesion and the change of neuromorphology. It is of great value to guide the development of surgical plan and evaluate the prognosis.
