目的研究外周血单个核细胞(PBMC)和血清中IL-35的水平与乙型肝炎疫苗免疫效果的关系,探讨影响乙型肝炎疫苗免疫应答的可能原因。方法随访400名按标准方案接种乙型肝炎疫苗后的健康大学生志愿者,化学发光法定量检测乙型肝炎病毒表面抗体(HBs Ab),随机选取无应答、弱应答组和中、强应答组各8人,分离PBMC,并提取总RNA,实时定量PCR检测构成IL-35的EB病毒诱导基因3(EBI3)和IL-12p35 mRNA表达水平;随机选取无应答、弱应答者和中、高应答者各40名,ELISA检测、比较2组人群血清中IL-35的含量,并与HBs Ab水平进行相关性分析。结果乙型肝炎疫苗免疫后中、强应答组PBMC中EBI3和IL-12p35 mRNA的含量显著高于无应答、低应答组,分别是无应答、低应答组的(12.73±2.26)倍和(7.93±1.06)倍;另外,中、强应答组人群血清中IL-35平均含量为(111.50±49.53)ng/L,显著高于无应答、弱应答组的(15.01±12.82)ng/L;血清HBs Ab水平与IL-35含量呈正相关(r=0.84)。结论外周血中IL-35水平与机体对乙型肝炎疫苗的免疫效果有关,IL-35水平低下可能是乙型肝炎疫苗无应答、弱应答发生的原因或机制之一。
Objective To study the relationship between the level of IL-35 in peripheral blood mononuclear cells (PBMCs) and serum and the immune effect of hepatitis B vaccine and to explore the possible causes that influence the immune response of hepatitis B vaccine. Methods A total of 400 volunteers were enrolled in this study. Healthy volunteers were enrolled in this study. The chemiluminescence method was used to detect HBsAb in hepatitis B virus (HBsAg) patients. Randomly selected nonresponders, weak responders and moderate responders 8, PBMCs were isolated and the total RNA was extracted. The EBV-induced gene 3 (EBI3) and IL-12p35 mRNA levels of IL-35 were detected by real-time quantitative PCR. Randomly selected nonresponders, The serum levels of IL-35 in the two groups were compared by ELISA, and the correlation was analyzed with the level of HBs Ab. Results The levels of EBI3 and IL-12p35 mRNA in the PBMCs of the strong response group were significantly higher than those in the non-response and low response groups (12.73 ± 2.26) and (7.93 ± 1.06) times. In addition, the average level of IL-35 in serum of moderate and strong responders was (111.50 ± 49.53) ng / L, which was significantly higher than that of no responders and weak responders (15.01 ± 12.82) ng / L; The level of HBs Ab was positively correlated with the level of IL-35 (r = 0.84). Conclusion The level of IL-35 in peripheral blood is related to the immune effect of hepatitis B vaccine. The low level of IL-35 may be the reason or mechanism of non-response or weak response of hepatitis B vaccine.
