目的探讨脂联素(Adiponectin,ADPN)在患者颈动脉斑块组织中的表达水平及其临床意义。方法收集2014年1月—2016年3月期间外科行颈动脉内膜剥脱术治疗的32例患者,切除的颈动脉斑块标本作为治疗组(A组)。按照Virmani颈动脉斑块分类方法将颈动脉斑块病理学分为:Ⅲ型以下5例(A1组),Ⅳ~Ⅴ型8例(A2组),Ⅵ~Ⅶ型19例(A3组)。21例腹部外伤切除的肠系膜上动脉标本作为对照组(B组)。采用实时荧光定量逆转录聚合酶链反应(RT-q PCR)技术及免疫荧光方法检测颈动脉斑块组与对照组标本中脂联素(ADPN)的表达情况,采用Spearman相关分析法分析颈动脉斑块组织中ADPN的表达水平与颈动脉斑块病理学的分型之间的相关性。结果 (1)治疗组中ADPN的表达水平[0.34(0.013,0.057)]显著低于对照组[0.76(0.347,0.429)];(2)32例治疗组患者中,与A1组[0.42(0.241,0.632)]相比,A2组[0.31(0.136,0.552)]颈动脉斑块组织中ADPN的表达显著下调,A3组[0.19(0.034,0.644)]下降最为显著,与A1、A2组差异均有统计学意义(P<0.05);(3)病理分型级别高颈动脉斑块中的ADPN表达呈低水平,两者之间具有负相关(r=-0.53,P=0.012)。结论 ADPN在颈动脉斑块中异常表达的严重程度呈负相关,即颈动脉斑块病理分型级别越高,ADPN相对表达越低。
Objective To investigate the expression of Adiponectin (ADPN) in carotid plaque and its clinical significance. Methods Thirty-two patients undergoing carotid endarterectomy from January 2014 to March 2016 underwent carotid plaque resection (A group). According to the classification of carotid plaques by Virmani, carotid plaque pathology was divided into 5 cases (group A1), 8 cases (group A2) and 19 cases (group A3) from group Ⅳ to group Ⅴ. Twenty-one cases of superior mesenteric artery resected with abdominal trauma were used as control group (group B). Adiponectin (ADPN) expression in carotid plaque group and control group was detected by real-time fluorescent quantitative reverse transcription polymerase chain reaction (RT-q PCR) and immunofluorescence method. Spearman correlation analysis was used to analyze carotid artery Correlation between ADPN expression levels in plaque and carotid plaque pathology. Results: (1) ADPN expression level in the treatment group [0.34 (0.013,0.057)] was significantly lower than that in the control group [0.76 (0.347,0.429)]; (2) Among the 32 treatment groups, , 0.632)], the ADPN expression in carotid plaque was significantly decreased in A2 group [0.31 (0.136,0.552)], and the decrease in A3 group [0.19 (0.034,0.644)] was the most significant (P <0.05). (3) ADPN expression in pathological type high carotid artery plaque was negative, with negative correlation (r = -0.53, P = 0.012). Conclusion ADPN is negatively correlated with the severity of abnormal expression in carotid plaque. The higher the pathological type of carotid plaque, the lower the relative expression of ADPN.
