目的检测慢性阻塞性肺疾病急性加重期(AE-COPD)患者呼吸道内具核梭杆菌(Fn.)的定植,探讨Fn.与AE-COPD间的关系。方法选择2008年10月至2009年4月中国医科大学附属第一医院、盛京医院确诊为AE-COPD的患者53例,采集呼吸道分泌物,提取细菌DNA,应用SYBR@GreenⅠPremix Ex Taq TM模式的实时PCR技术,对Fn.进行定量检测,并计算其所占总菌的比例。同时记录患者FEV1占预计值比例(%)及口腔牙周指标。结果 53例样本中有32例检出Fn.,Fn.检出率为60.38%,占总菌的比例为(44.58±16.47)%。Fn.相对含量与FEV1占预计值比例呈负相关关系(P<0.05)。Fn.相对含量与简化口腔卫生指数及附着丧失水平呈正相关关系(P<0.05);与牙龈出血指数、探诊深度、缺失牙数无相关性(P>0.05)。结论 AE-COPD患者呼吸道内存在Fn.,Fn.相对含量随着肺功能的减弱而增加;Fn.与AE-COPD密切相关。
Objective To investigate the colonization of Fn in the respiratory tract of patients with acute exacerbation of chronic obstructive pulmonary disease (AE-COPD) and to explore the relationship between Fn and AE-COPD. Methods 53 patients diagnosed as AE-COPD from the First Affiliated Hospital of China Medical University from October 2008 to April 2009 were collected respiratory secretions, bacterial DNA was extracted, using SYBR @ Green I Premix Ex Taq TM mode Real-time PCR technology, quantitative detection of Fn., And calculate the proportion of total bacteria. At the same time, the ratio of FEV1 in patients with the predicted value (%) and oral periodontal index were recorded. Results Of the 53 samples, 32 cases were detected with Fn., Fn. The detection rate was 60.38%, accounting for (44.58 ± 16.47)% of total bacteria. The relative content of FV was negatively correlated with the ratio of FEV1 to the expected value (P <0.05). There was a positive correlation between the relative content of oral hygiene index and simplified oral health index and attachment loss (P <0.05), but no correlation with gingival bleeding index, probing depth and missing teeth (P> 0.05). Conclusions There is Fn, Fn in the respiratory tract of patients with AE-COPD, the relative content of which increases with the decrease of pulmonary function, and Fn is closely related to AE-COPD.
