肺结核合并恶性淋巴瘤临床较少见.我院1882年3月～1992年10月共收集3027例各种肺结核病例.合并各种肿瘤169例,占 5.58%.其中11例合并恶性淋巴瘤占所有肿瘤病例的6.5%.报道如下.1 临床资料11例中男8例,女3例、年龄21～74岁.平均45.1岁.浸润性肺结核5例,支气管内膜结核1例.慢性纤维空洞型肺结核4例,结核性胸膜炎1例.合并何杰金氏淋巴瘤5例,非何杰金氏淋巴瘤6例.11例均经过手术、活检病理诊断,有6例肺结核与合并恶性淋巴瘤病程基本一致,5例肺结核病程8个月～10年,经各种抗结核治疗迁延来愈,后合并恶性淋巴瘤11例全部随访,治疗后存活时间5～39个月,平均23个月.合并各种淋巴瘤原发于淋巴结者5例,淋巴结外6例,其中鼻腔2例.纵隔、胃、咽淋巴环、腹腔淋巴结各1例.首发症状贫血6例,发热、盗汗9例,持续性高热2例.血红蛋白74～114g/L,平均86g/L,血沉23～92mm/h,平均43mm/h.痰结核杆菌阳性率100%.旧结核菌素试验(OT)1:10000阴性率100%、1:2000阴性率45.6%.IgG3、2～7.3g/L,平均4.8g/L.恶性淋巴瘤病理分型,淋巴细胞为主型2例,混合细胞型1例,结节硬化型2例.非何杰金氏淋巴瘤, 低恶度1例,中恶度3例,高恶度2例.
