目的探讨低熔点铅不规则挡块技术放射治疗鼻咽癌的近期疗效及近期并发症。方法129例鼻咽癌患者,随机分为实验组和对照组,其中实验组66例,采用低熔点铅不规则挡块技术放疗;对照组63例,采用常规挡块技术放疗,6MV-X线照射鼻咽治疗量为66~70Gy,颈部治疗量DT60~70Gy,颈部预防量为DT50Gy。结果实验组和对照组的鼻咽部肿瘤缓解率(CR+PR)分别为100%、100%;两组患者1、2年肿瘤局部控制率分别98.45%、95.45%和98.35%、93.65%(P>0.05),两组患者1、2年生存率分别为98.48%、89.32%和98.35%、90.99%(P>0.05)。对照组患者放疗期间严重急性黏膜炎、咽痛和放疗后重度口干发生率分别为46.0%、36.5%、49.2%,而实验组分别为25.8%、13.6%、9.1%(P<0.05)。结论低熔点铅不规则挡块技术能有效地保护部分正常组织,减少急性放疗反应。
Objective To investigate the short-term curative effect and short-term complication of nasopharyngeal carcinoma treated by low-melting lead irregular stop technology. Methods A total of 129 patients with nasopharyngeal carcinoma were randomly divided into experimental group and control group, of which 66 cases in the experimental group received radiotherapy with low-melting lead irregular block technique. In the control group, 63 cases received conventional block radiotherapy and 6MV-X Radiation nasopharyngeal treatment volume of 66 ~ 70Gy, the amount of neck treatment DT60 ~ 70Gy, prevention of the amount of neck DT50Gy. Results The CR + PR of the experimental group and the control group were 100% and 100%, respectively. The local control rates at 1 and 2 years of the two groups were 98.45%, 95.45% and 98.35% and 93.65% P> 0.05). The 1-year and 2-year survival rates of the two groups were 98.48%, 89.32% and 98.35%, 90.99%, respectively (P> 0.05). Severe acute mucositis, sore throat and severe dry mouth incidence after radiotherapy were 46.0%, 36.5% and 49.2% respectively in the control group, compared with 25.8%, 13.6% and 9.1% in the experimental group (P <0.05). Conclusion Low-melting lead irregular stop technology can effectively protect some normal tissues and reduce the acute radiation response.
