目的:评价分诊断单元服务模式在减少医院儿科药房内部调剂差错事件方面的成效。方法:采用回顾性研究方法,对东莞市第五人民医院儿科药房2015年登记的71例内部调剂差错进行分类统计,分析原因,评价分诊断单元服务模式的实施成效,持续改进服务质量。结果:东莞市第五人民医院儿科药房在传统服务模式下(2015年1-6月)发生内部调剂差错事件53例,在分诊断单元服务模式下(2015年7-12月)发生内部调剂差错事件18例,差错率由0.540‰下降到0.182‰(P<0.01)。儿科药房内部调剂差错事件的原因主要分为人员因素、环境因素、药品因素和工作流程因素4个方面,可从这4个方面着手持续改进服务质量。结论:分诊断单元服务模式在减少儿科药房内部调剂差错事件方面具有良好的应用前景。
OBJECTIVE: To evaluate the effectiveness of subdivision unit service model in reducing incidental errors in hospital pediatric pharmacies. Methods: A retrospective study was conducted to classify 71 cases of internal transfers registered in Pediatric Pharmacy of Dongguan Fifth People ’s Hospital in 2015, analyze the causes, evaluate the effectiveness of the service mode of diagnostic units and continuously improve the quality of service. Results: In the traditional service mode (January-June 2015), 53 pediatric pharmacies in the Fifth People’s Hospital of Dongguan City had internal adjustment errors, and internal adjustment errors occurred in the service mode of diagnostic units (July-December 2015) In 18 cases, the error rate decreased from 0.540 ‰ to 0.182 ‰ (P <0.01). The reasons for errors in the internal dispensary of pediatric pharmacy are mainly divided into four aspects: personnel factor, environmental factor, pharmaceutical factor and work flow factor, and the quality of service can be continuously improved from the four aspects. Conclusion: The sub-diagnosis unit service model has a good application prospect in reducing the error of internal adjustment of pediatric pharmacy.
