目的探讨视黄醇(维生素A)结合蛋白(RBP4)基因多态性与老年人糖尿病合并冠心病的关系。方法选取老年人糖尿病患者230名,其中糖尿病合并冠心病患者129名,并与125名健康老年人相对比。检测身高、体重、腹围、臀围、心电图、血压、血脂、空腹血糖(FPG)、空腹胰岛素(FINS)、计算胰岛素抵抗指数(HOMA-IR)。采用聚合酶链式(PCR)-变性高效液相色谱(DHPLC)技术筛查RBP4基因第五内含子的部分序列,测序分析结果。结果老年人糖尿病合并冠心病组与单纯糖尿病组相比,受试对象的总胆固醇水平及低密度胆固醇水平均增高(P<0.05)。老年人群中RBP4基因+7542(T>D)核酸位点有三种基因型:T/T型、T/D型、D/D型。老年人糖尿病合并冠心病组的TT型基因频率高于其它组。结论 RBP4基因多态性可能与老年人糖尿病合并冠心病相关。
Objective To investigate the relationship between retinol (vitamin A) binding protein (RBP4) gene polymorphism and coronary heart disease in elderly with diabetes mellitus. Methods A total of 230 elderly diabetic patients were selected, of whom 129 were diabetic patients with coronary heart disease and were compared with 125 healthy elderly people. Body height, weight, abdominal circumference, hip circumference, electrocardiogram, blood pressure, blood lipid, fasting blood glucose (FPG) and fasting insulin (FINS) were measured to calculate the insulin resistance index (HOMA-IR). The partial sequence of the fifth intron of RBP4 gene was screened by polymerase chain reaction-denaturing high performance liquid chromatography (DHPLC) and sequenced. Results Compared with the diabetic group, the total cholesterol and low density cholesterol of the elderly patients with diabetes mellitus and coronary heart disease were significantly increased (P <0.05). There are three genotypes of RBP4 +7542 (T> D) nucleic acid sites in the elderly population: T / T, T / D and D / D. The frequency of TT genotypes in elderly diabetic patients with coronary heart disease was higher than that in other groups. Conclusion RBP4 gene polymorphism may be associated with diabetes mellitus in elderly patients with coronary heart disease.
