目的:探讨阿卡波糖联合苯磺酸左旋氨氯地平对高血压并糖耐量减低(impaired glucose tolerance,IGT)患者肱-踝脉搏波速度(brachio-ankle pulse wave velocity,baPWV)的影响。方法:选择高血压并IGT患者59例,随机分为苯磺酸左旋氨氯地平组(B组)30例,阿卡波糖加苯磺酸左旋氨氯地平组(C组)29例,对照组30例(A组);观察治疗前后血压、血糖、一氧化氮、内皮素(endothelin,ET)及baPWV的变化。结果:治疗6个月后,B组及C组的血压、ET水平及baPWV较治疗前显著下降(P<0.01),NO水平显著上升(P<0.01),C组的餐后2h血糖(2-hour postprandial blood glucose,2hPG)较治疗前显著下降(P<0.01)。两组治疗后,C组的2hPG、ET水平及baPWV明显低于B组(P<0.05),NO水平高于B组(P<0.05)。结论:阿卡波糖和苯磺酸左旋氨氯地平都可以降低高血压并IGT患者的baPWV,改善大动脉的弹性及顺应性。
Objective: To investigate the effect of acarbose combined with levoamlodipine besylate on brachio-ankle pulse wave velocity (baPWV) in patients with hypertension and impaired glucose tolerance (IGT). Methods: Fifty-nine patients with hypertension and IGT were randomly divided into 30 cases in group B (levamlodipine besylate group), 29 cases in acarbose plus levamlodipine besylate group (group C) (Group A). The changes of blood pressure, blood glucose, nitric oxide, endothelin (ET) and baPWV were observed before and after treatment. Results: The blood pressure, ET level and baPWV in group B and group C were significantly lower than those before treatment (P <0.01), while the level of NO was significantly increased in group B and C (P <0.01) -hour postprandial blood glucose, 2hPG) was significantly lower than before treatment (P <0.01). After treatment, the levels of 2hPG, ET and baPWV in group C were significantly lower than those in group B (P <0.05), and the levels of NO were higher in group C than those in group B (P <0.05). CONCLUSION: Both acarbose and levamlodipine besylate can reduce baPWV in hypertensive patients with IGT and improve aortic elasticity and compliance.
