目的:评价医疗信息云平台用于日间手术管理的价值。方法:择期妇科日间手术患者,年龄≥18岁、ASA分级Ⅰ或Ⅱ级,采用随机数字表法分为2组:云平台管理组(P组)和传统管理组(C组)。P组采用医疗信息云平台辅助管理,患者宣教及随访均在云平台完成;C组采用医护口头宣教、发放宣教材料及电话随访。于手术日晨,采用问卷法确定宣教效果评分;采用焦虑自评量表确定术前焦虑程度评分;测量血压及心率,计算手术日较门诊日水平的变化率。记录日间手术延迟-取消、术后24 h随访、术后并发症发生及患者主动反馈的情况;记录医护围术期宣教时间及随访时间。结果:本研究共纳入226例妇科日间手术患者。与C组(n n=113)比较,P组(n n=113)术前宣教效果评分升高,术前焦虑程度评分、血压变化率降低,手术延迟-取消率显著降低(6.2%/0)(n P0.05),而术后主动反馈率明显升高,医护围术期宣教时间及随访时间明显缩短(n P<0.05)。n 结论:医疗信息云平台不仅可改善围术期宣教和术后随访的质量,还可提高其效率,对完善日间手术管理具有显著的临床价值。“,”Objective:To evaluate the value of cloud medical health information platform for management of day surgery.Methods:The patients, aged≥18 yr, of American Society of Anesthesiologists physical statusⅠ or Ⅱ, scheduled for elective gynecological day surgery, were divided into 2 groups using a random number table method: cloud platform management group (group P) and conventional management group (group C). In group P, the cloud medical health information platform was used to assist patient management, and patient education and follow-up both were completed on the cloud platform.In group C, medical staff carried out patient management through oral health education, leaflets and telephone follow-up.On the morning of the operation day, questionnaires were given out to patients to evaluate the effect of patient education which were scored; Self-rating Anxiety Scale was used to evaluate preoperative anxiety; blood pressure and heart rate were measured, and the changing rate between the operation day and the outpatient day was calculated.The day surgery delays-cancellations, follow-up of 24 h after operation, postoperative complications and active feedback of patients were recorded.The time spent on patient education by medical staff during the perioperative period and follow-up time were recorded.Results:Two hundred and twenty-six patients undergoing elective gynecological day surgery were enrolled in this study.Compared with group C (n n=113), the score for the effect of patient education was significantly increased, the score for preoperative anxiety and changing rate of blood pressure were decreased, the rate of surgery delays-cancellations was decreased (6.2%/0) (n P<0.05), no significant change was found in the rate of postoperative follow-up and incidence of complications, the rate of postoperative active feedback was increased, and the time spent on patient education by medical staff during the perioperative period and follow-up time were shortened in group P (n n=113) (n P<0.05).n Conclusion:The cloud medical health information platform can not only improve the quality of perioperative patient education and postoperative follow-up, but also raise the efficiency, which has significant clinical value in improving management of day surgery.
