目的探讨左心房消融治疗心房颤动的临床疗效。方法对3例心房颤动患者采用非接触心内膜激动标测系统(EnSite3000)建立左心房、肺静脉的三维等时电势图和电解剖图,在距离肺静脉口1~2cm处于左房后壁描记左上、下肺静脉为一个消融隔离环线,描记右上、下肺静脉为一个消融隔离环线、左房后顶部两环之间为一个消融隔离线,左环至二尖瓣峡部连线作为消融隔离线。于非X线下分别沿各线依次消融。结果 3例患者房颤全部终止,术后6个月内均未复发房颤,无1例患者出现并发症,平均手术时间(82.00±36.10)分钟,平均X线曝光时间(20.00±6.24)分钟。结论非接触心内膜标测系统指导下的左房消融是治疗阵发性房颤的有效而安全的方法。
Objective To investigate the clinical effect of left atrial ablation on atrial fibrillation. Methods Three-dimensional isokinetic potential maps and electrical anatomy of left atrium and pulmonary veins were established by using non-contact endocardial activation mapping system (EnSite3000) in 3 patients with atrial fibrillation. Left to right posterior wall tracts were located 1 ~ 2 cm away from the pulmonary veins , The lower pulmonary vein as an ablation isolation ring, tracing the right upper and lower pulmonary veins as an ablation isolation loop, left abdomen after the top two rings as an ablation isolation line, the left ring to the mitral valve isthmus ablation line as ablation. In the non-X-ray respectively along the line followed by ablation. Results Atrial fibrillation was terminated in all 3 patients. No recurrence of atrial fibrillation occurred within 6 months after operation. No patient developed complications. The average operation time was 82.00 ± 36.10 minutes and the average X-ray exposure time was 20.00 ± 6.24 minutes . Conclusion Non-contact endocardial mapping system under the guidance of left atrial ablation is an effective and safe treatment of paroxysmal atrial fibrillation.
