[目的]了解上消化道癌高危个体参与筛查的依从性的影响因素。[方法]40~69岁上消化道癌高危个体367例,包括胃癌高危个体259例和食管癌高危个体108例;参与进一步筛查完全依从者234例,不依从者133例。采用问卷调查方式调查筛查的依从性。[结果]多因素Logistic回归分析显示:对筛查目的认识不清、学历低和肿瘤家族史可能是影响上消化道癌高危个体低依从性的主要因素。清楚筛查目的个体参与筛查的可能性为不了解筛查目的个体的1.786倍(95%CI:1.115~2.861)。[结论]对筛查目的的了解、文化程度和肿瘤家族史是影响高危个体参与筛查依从性的主要因素。应侧重于文化程度低和无肿瘤史人群的健康教育,做好医务人员与筛查高危个体沟通工作,提高人群参与筛查的依从性。
[Objective] To investigate the influencing factors of compliance of high risk individuals involved in screening for upper gastrointestinal cancer. [Methods] A total of 367 high risk individuals with upper gastrointestinal cancer aged 40-69 years were enrolled, including 259 high risk individuals with gastric cancer and 108 high risk individuals with esophageal cancer; 234 patients were further screened for complete compliance and 133 noncompliant patients. Use questionnaires to investigate compliance with screening. [Results] Multivariate Logistic regression analysis showed that the lack of understanding of the purpose of screening, low educational level and family history of tumor may be the main factors that affect the low compliance of individuals with high risk of upper gastrointestinal cancer. Clear screening purposes The probability of individuals participating in screening is 1.786 times (95% CI: 1.115 ~ 2.861) of individuals who do not know the purpose of screening. [Conclusion] Knowledge of screening purpose, educational level and family history of tumor are the main factors influencing screening compliance of high risk individuals. Should focus on the education of people with low educational level and no history of cancer, medical personnel and screening high-risk individuals to communicate, improve the compliance of the crowd to participate in screening.
