目的评价HbA2和HbF检测在儿童β-地中海贫血(β-地贫)筛查中的应用价值。方法选取β-地中海贫血患儿98例以及健康儿童105例作为研究对象,分别检测血浆中Hb A2、HbF含量。结果β-地贫组HbA2、Hb F水平明显高于对照组(P<0.01)。HbA2、HbF及联合诊断β-地贫受试者工作特征曲线下面积分别为0.921,0.900,0.951,cutoff值分别为3.3%,0.6%。Logistic回归分析表明,与HbF相比,HbA2诊断价值更大。结论 HbA2和HbF检测对β-地贫诊断具有较高的灵敏度和特异性,是大规模筛查β-地贫比较理想的方法;且HbA2的诊断价值优于HbF;HbA2和HbF联合检测可提高儿童β-地贫的诊断符合率。
Objective To evaluate the value of HbA2 and HbF detection in screening β-thalassemia in children. Methods A total of 98 children with β-thalassemia and 105 healthy children were enrolled in this study. The contents of Hb A2 and HbF in plasma were detected respectively. Results The levels of HbA2 and Hb F in β-thalassemia group were significantly higher than those in control group (P <0.01). The area under the working characteristic curves of HbA2, HbF and combined diagnosed β-thalassemia subjects were 0.921, 0.900 and 0.951 respectively, and the cutoff values were 3.3% and 0.6% respectively. Logistic regression analysis showed that HbA2 was more valuable than HbF in diagnosis. Conclusion The detection of HbA2 and HbF has high sensitivity and specificity for the diagnosis of β-thalassemia, which is an ideal method for large-scale screening of β-thalassemia. HbA2 is superior to HbF in the diagnosis of β-thalassemia. HbA2 and HbF combined detection can improve Diagnostic accuracy of β-thalassemia in children.
