目的对比人感染H7N9禽流感和甲型H1N1流感重症肺炎的CT影像特点。方法选取2013年5月至2015年3月收治的甲型H1N1流感合并重症肺炎患者20例。同时选取2013年11月至2015年3月确诊为人感染H7N9禽流感合并重症肺炎的15例患者。两组均行全面检查,对比两组检查结果。结果 H7N9患者ARDS发生率明显高于H1N1患者;H7N9患者可见的小叶间隔变厚、网格状密度影、胸腔积液表现明显高于H1N1患者;H7N9患者病死率明显高于对照组(P<0.05)。结论人感染H7N9禽流感合并重症肺炎比甲型H1N1流感合并重症肺炎病情进展更快,病死率更高,CT影像诊断有助于医生评估患者病情,及早发现并发症。
Objective To compare the CT features of H7N9 bird flu and Influenza A (H1N1) severe pneumonia. Methods Twenty patients with influenza A (H1N1) and severe pneumonia admitted from May 2013 to March 2015 were selected. Fifteen patients diagnosed as H7N9 bird flu with severe pneumonia were selected from November 2013 to March 2015. Two groups were comprehensive examination, comparing the two groups of test results. Results The incidence of ARDS in patients with H7N9 was significantly higher than that in patients with H1N1. The interlobular septa were more obvious in H7N9 patients than in H1N1 patients. The mortality of H7N9 patients was significantly higher than that of controls (P <0.05) ). Conclusion Human H7N9 avian influenza infection complicated with severe pneumonia progresses faster and has a higher case fatality rate than type A H1N1 influenza with severe pneumonia. CT diagnosis helps doctors assess the patient’s condition and detect complications early.
