社区护理是以社区人群为服务对象,对个人、家庭及社区提供健康、预防疾病、早期诊断、早期治疗、限制残障等服务,提高社区人群的健康水平.老年人是社区的特殊人群,由于年龄和生理特点,不同程度地存在健康问题.提高老年人健康水平是当今社会的重要研究课题.据2001年4月第5次全国人口普查结果表明,我国60岁以上老年人口已达1.32亿,并正以每年3.2%的速度增长.其中约80%的老年人患有至少一种慢性疾病.根据社区护理和社区护理干预的理念,我院自2001年开始,在医院周边的社区中,通过筛选对80名老年人开展了健康教育,
