目的探讨糖化血红蛋白A1(glycosylated hemoglobin,HbA1c)和血清胱抑素C(cystatin C,CysC)与2型糖尿病患者肾损害程度的关系。方法糖尿病肾病患者134例依据尿微量白蛋白水平分为3组,<30mg/d者28例为Ⅰ组,30~300mg/d者41例为Ⅱ组,>300mg/d者65例为Ⅲ组,同期体检健康者90例为对照组,测定各组血清CysC,HbA1c水平并进行比较。结果Ⅰ,Ⅱ,Ⅲ组血清HbA1c和CysC水平均高于对照组(P<0.05),Ⅰ组与Ⅱ和Ⅲ组,Ⅱ组与Ⅲ组比较差异均有统计学意义(P<0.05);糖尿病肾病患者血清HbA1c与CysC水平呈显著正相关(r=0.657,P<0.05)。结论 2型糖尿病患者HbA1c和CysC水平越高提示肾损害程度越严重。
Objective To investigate the relationship between glycosylated hemoglobin A1 (HbA1c) and cystatin C (CysC) and the degree of renal damage in type 2 diabetic patients. Methods 134 patients with diabetic nephropathy were divided into 3 groups according to urinary albumin level, 28 cases were group Ⅰ <30mg / d, 41 cases were group Ⅱ from 30 to 300mg / d, 65 cases were group Ⅲ> 300mg / d In the same period, 90 cases of healthy subjects were taken as control group. The levels of serum CysC and HbA1c in each group were measured and compared. Results The levels of serum HbA1c and CysC in group Ⅰ, Ⅱ and Ⅲ were significantly higher than those in control group (P <0.05). There was significant difference between group Ⅰ and group Ⅱ and Ⅲ, group Ⅱ and group Ⅲ (P <0.05) There was a significant positive correlation between HbA1c and CysC in patients with nephropathy (r = 0.657, P <0.05). Conclusions The higher HbA1c and CysC levels in type 2 diabetic patients indicate that the more severe the renal damage is.
