目的 :研究射频多极针对肺癌的治疗作用。方法 :采用WE75 6 8射频多极针肿瘤消融仪 ,对 4 2例肺癌的病人 ,经胸腔镜或开放手术直视下或CT定位下行肺癌肿块消融术 ,2 5例消融术后即行病灶肺叶切除术 ,病灶送病理 ,观察消融对肺癌细胞作用后的病理变化和该术式的不良反应。结果 :消融后大部分肿瘤可见较多片状 ,小灶性坏死 ,在坏死中间及边缘可见部分癌细胞核固缩 ,核分裂 ,部分空泡变性 ,嗜酸性变。部分癌细胞巢的中央见细胞松懈、脱落 ,或见纤维素样坏死或癌细胞广泛变形。从坏死边缘残留的癌细胞巢到正常肺组织距离为 0 .0 1~ 0 .2cm左右 ,坏死灶中间及边缘有大量中性白细胞渗出 ,低分化与中分化癌细胞对温度耐受无明显差别。结论 :带有射频多极针的WE75 6 8肿瘤消融仪 ,对肺癌治疗具有较好的近期疗效 ,能够使肿瘤病灶大部分坏死 ,明显地减缓肿瘤的发展 ,减少疼痛 ,有效地改善患者的生活质量 ,但目前仍未证实其具有根治作用
Objective: To study the therapeutic effect of multi-pole radiofrequency on lung cancer. Methods: Forty-two patients with lung cancer underwent radiofrequency ablation using a WE75 6 8 radiofrequency multipolar tumor ablation instrument. Thoracotomy or open surgery under direct vision or CT angiography was performed for ablation of lung cancer. Results: Most of the tumors showed more lamellar and focal necrosis after ablation. Some of the cancer cells were nuclear pyknosis, mitotic degeneration and eosinophilic alteration in the middle of the necrosis. Part of the center of the nests of cancer cells, see cell loosening, loss, or see cellulose-like necrosis or extensive deformation of cancer cells. From the edge of necrotic cancer cells nest to the normal lung tissue distance of 0. 01 ~ 0. 2cm or so, a large number of neutrophil exudation in the middle and marginal necrosis, poorly differentiated and moderately differentiated cancer cells had no significant resistance to temperature difference. CONCLUSION: The WE75 6 8 tumor ablation instrument with RF multi-pole needle has good short-term curative effect on the treatment of lung cancer, which can cause most of the tumor lesions to be necrotic, obviously slow down the development of the tumor, reduce the pain and effectively improve the life of the patient Quality, but it has not yet confirmed its radical effect
