目的分析衢州市涂阳肺结核患者结核分枝杆菌的耐药情况与耐药谱。方法对2010年3月—2013年3月在衢州市6家结核病定点医院就诊的1976例涂阳肺结核患者的痰进行结核分枝杆菌培养,用比例法对6种抗结核药物进行耐药测试。结果 1335株结核分枝杆菌中,总耐药率为22.02%,初始耐药率和获得性耐药率分别为18.25%和52.74%;总耐多药率为4.60%,初始耐多药率和获得性耐多药率分别为1.18%和32.19%,广泛耐多药率为0.37%。单耐药谱顺序为SM>OFX>INH,多耐药谱顺序为INH+SM>INH+SM+OFX>INH+OFX(RFP+OFX),耐药谱顺序为INH+RFP+SM>INH+RFP+SM+OFX>INH+RFP。结论衢州市结核病耐药率低于全国平均水平,但耐多药发生率较高。
Objective To analyze the drug resistance and drug resistance of Mycobacterium tuberculosis in smear-positive pulmonary tuberculosis patients in Quzhou. Methods Mycobacterium tuberculosis was cultured in sputum of 1976 smear-positive pulmonary tuberculosis patients in 6 TB designated hospitals in Quzhou City from March 2010 to March 2013. Drug resistance of 6 anti-tuberculosis drugs was tested by proportional method. Results The total drug resistance of 1335 strains of Mycobacterium tuberculosis was 22.02%, the initial drug resistance rate and the acquired drug resistance rate were 18.25% and 52.74% respectively, the total MDR rate was 4.60%, the initial MDR rate and The acquired multidrug resistance rates were 1.18% and 32.19%, respectively, and the broad multidrug resistance rate was 0.37%. The order of single resistance spectrum was SM> OFX> INH, the order of multi-drug resistance was INH + SM> INH + SM + OFX> INH + OFX RFP + SM + OFX> INH + RFP. Conclusion The resistance rate of TB in Quzhou City is lower than the national average, but the incidence of MDR is higher.
