目的:观察Neuro-Endoport技术辅助神经内镜治疗脑室内肿瘤的临床效果。方法:回顾性分析2018年1月至2019年12月首都医科大学附属北京天坛医院神经外科采用Neuro-Endoport技术辅助神经内镜治疗脑室内肿瘤患者的临床资料，共15例。观察其手术时间、肿瘤切除程度、术中出血量、术后并发症及随访结果。结果:15例患者中，肿瘤全切除12例，近全切除3例。手术时间为2.5～7.0(4.2±1.6)h，术中出血量为100～1 000(352±73)ml；术中造瘘10例；术中均无并发症发生。住院天数为8～20(12.3±4.3)d；术后发生颅内感染2例、脑水肿4例、癫痫发作2例、脑积水1例。15例患者的随访时间为3～12(7.9±2.3)个月。随访期间10例术前合并脑积水的患者症状明显缓解，2例癫痫发作者未再发作，4例脑水肿和2例颅内感染者均恢复良好；头颅MRI显示，10例造瘘患者的脑室均不同程度地缩小，4例未行造瘘的患者未出现明显脑积水，1例术后脑积水患者的脑室较出院前缩小。所有患者的肿瘤均无复发。结论:Neuro-Endoport技术辅助神经内镜手术治疗脑室内肿瘤术中并发症发生率低，术后疗效好。“,”Objective:To explore the efficacy of neuroendoscopic treatment of intraventricular tumors assisted with Neuro-Endoport technology.Methods:A retrospective study was conducted on the clinical data of 15 patients with intraventricular tumors who underwent neuroendoscopic treatment assisted with Neuro-Endoport technology from January 2018 to December 2019 at Department of Neurosurgery, Beijing Tiantan Hospital, Capital Medical University. We documented the operation duration, tumor resection degree, intraoperative blood loss, postoperative complications and follow-up results.Results:Of the 15 patients, 12 had total tumor resection and 3 had near-total resection. The operation duration was 2.5-7.0 (4.2±1.6) hours, and intraoperative blood loss was 100-1 000 (352±73) ml. Ventriculostomy was performed during operation in 10 cases. No intraoperative complications occurred. The length of hospital stay was 8-20 (12.3±4.3) days. There were two cases of intracranial infection, four cases of cerebral edema, two cases of seizures, and one case of hydrocephalus after operation. The follow-up duration of 15 patients was 3-12 (8±2) months. During the follow-up period, the symptoms of 10 patients with preoperative hydrocephalus were significantly relieved; two patients with previous seizures did not report new events of seizures; four patients with cerebral edema and two patients with intracranial infection recovered well; head MRI showed that the ventricles of 10 patients with ventriculostomy shrank to varying degrees, four patients without ventriculostomy showed no obvious hydrocephalus, and one patient with postoperative hydrocephalus had a smaller ventricle than that before discharge. None of the patients had tumor recurrence.Conclusion:Neuroendoscopic surgery assisted with Neuro-Endoport technology seems safe and effective for the treatment of intraventricular tumors.
