目的:探讨转矩力对骨性上颌前突病例上颌切牙整体移动的效果及对侧貌的影响。方法:实验组16例骨性上颌前突患者,年龄14~34岁(平均26岁),常规上颌拔牙矫治内收前牙,用0.019“×0.025”不锈钢方丝弯制标准弓形使上颌切牙段根舌向转矩30°控根;对照组16例骨性上颌前突病例,年龄12~45岁(平均32岁),拔牙矫治内收上颌前牙,未做控根处理。矫治前后的X线头颅定位侧位片比较切牙的轴倾度和面部软组织的变化。结果:实验组∠SNB、∠ANB、U1-SN、U1-NA、NLA、UL-EL、Facial Convexity的变化大于对照组,差别均有统计学意义(P<0.001)。结论:上颌骨性前突病例内收前牙后控根可避免切牙过度舌倾,上颌骨基骨后移,上颌前突面型改变更加明显。
Objective: To investigate the effect of torque on the overall movement of the maxillary incisors in patients with bony maxillary protrusion and its effect on the lateral appearance. Methods: Twenty - six patients with bony maxillary protrusion in the experimental group, aged 14-34 years old (average 26 years), were treated with maxillary extraction and maxillary extraction. The maxillary mandibular curvature was obtained with a 0.019 “× 0.025” stainless steel square wire The control group had 16 cases of bony maxillary protrusion, aged from 12 to 45 years old (average 32 years old). The maxillary anterior teeth were extracted by tooth extraction and the root control was not performed. X-ray cranial positioning before and after treatment of lateral radiographs to compare incisors axial inclination and facial soft tissue changes. Results: The changes of ∠SNB, ∠ANB, U1-SN, U1-NA, NLA, UL-EL and Facial Convexity in the experimental group were significantly higher than those in the control group (P <0.001). CONCLUSIONS: The maxillary protrusion of the anterior teeth in the maxillary odontoid cases can prevent the over-tongue incisor, the maxillary basement bone from moving backward, and the maxillary protrusion face shape change more obviously.
