近十年来美国的HMO(健康维护组织)和Medicare(老年保险)采用向医院实行预付款方法后,医院的行政管理人员和医务人员间形成相互依赖的利害关系。临床工作是决定医院财政情况的唯一重要因素。医院是一个多科性机构,诊治病人需要医师、护士、行政和后勤人员共同努力。每个人员都化费医院的财力,所以在管理中要确定每人的职责,并考核其工作效率和效益。任何一个部门工作不当,都会引起医院的经济损失。本文介绍密执安大学医院近2年采用的住院病人综合管理方案。
In the past decade, the adoption of prepayment methods for hospitals by HMOs (Health Maintenance Organizations) and Medicare (Old Age Insurance) in the United States has resulted in interdependent relationships between hospital administrators and medical personnel. Clinical work is the only important factor in determining the financial status of a hospital. The hospital is a multi-disciplinary institution. The diagnosis and treatment of patients requires the joint efforts of physicians, nurses, administrative and logistics personnel. Each person spends money on the hospital’s financial resources. Therefore, in the management, it is necessary to determine the responsibilities of each person and to assess their efficiency and effectiveness. Improper work in any department will cause economic losses in the hospital. This article describes the integrated management of inpatients used by the University of Michigan Hospital in the last two years.
