目的 探讨不孕症患者采用克罗米芬(CC) 兴奋试验,预测卵巢储备能力。方法 通过54 例不孕症妇女月经第3 天测定血卵泡刺激素(FSH)、雌二醇(E2)水平,月经第5~9 天口服CC100mg/d,第10 天再次测定FSH、E2 ,B超跟踪监测卵泡发育及妊娠情况。结果 35 岁以上和以下两组患者基础FSH、CC兴奋试验和妊娠率均有显著差异。年龄越大,基础FSH值越高,CC兴奋试验反应率越低,妊娠率越低。本组54 例采用改良CC兴奋试验有反应36 例,反应不良18 例,分别采用CC、CC/hMG、hMG、FSH 促排卵,共86 个周期,妊娠14 例,周期妊娠率分别为16-67% 、12-50 % 、40 % 、100 % 。结论 采用改良CC 兴奋试验预测卵巢储备能力有更高的筛选价值,并指导卵巢有控制性过度刺激(COH)方案的选择,有效提高妊娠率。
Objective To investigate the infertility patients using clomiphene citrate (CC) excitability test to predict ovarian reserve capacity. Methods FSH and E2 levels were measured on the 3rd day of menstruation in 54 cases of infertility women. CC100mg / d was given orally on the 5th to 9th day after menstruation. The levels of FSH, E2, B Ultra-tracking monitors follicular development and pregnancy status. Results There were significant differences in basal FSH, CC excitability test and pregnancy rate between the age of 35 and below. The older, the higher the basal FSH value, the lower the response rate of CC excitement test, the lower the pregnancy rate. The group of 54 patients with modified CC excitement test response 36 cases, 18 cases of adverse reactions were used CC, CC / hMG, hMG, FSH ovulation, a total of 86 cycles, 14 cases of pregnancy, the cycle of pregnancy rates were 16-67 %, 12-50%, 40%, 100%. Conclusion The modified CC excitability test predicts the ovarian reserve capacity has a higher screening value, and guide the choice of ovarian controlled over-stimulation (COH) program, effectively improve the pregnancy rate.
