目的评价舒张性心功能衰竭(DHF)患者血浆脑钠肽(BNP)、醛固酮(ALD)和肌钙蛋白Ⅰ(c Tn I)水平的检测意义。方法随机选取2014年5月-2015年5月在本院住院的DHF患者共181例,根据NYHA心功能分级标准分为心功能2级组39例,3级组77例,4级组65例;另选同期本院120例健康体检者,作为正常对照组。采用ELISA法检测BNP水平,采用放射免疫法检测ALD水平,采用免疫定量比浊法检测cTnⅠ水平。结果 DHF组较正常对照组,心功能3级组、心功能4级组较心功能2级组,心功能4级组较心功能3级组,血浆BNP、ALD和c Tn I水平均明显升高,差异有统计学意义(P<0.01)。相关性分析表明血浆BNP与ALD和c Tn I水平分别呈显著正相关(r=0.614,P<0.05;r=0.586,P<0.05)。结论检测血浆BNP、ALD和c Tn I水平有助于诊断DHF,评估其严重程度,以及评价其近期治疗效果。
Objective To evaluate the significance of plasma brain natriuretic peptide (BNP), aldosterone (ALD) and troponin I (cTn I) levels in patients with diastolic heart failure (DHF). Methods A total of 181 DHF patients hospitalized in our hospital from May 2014 to May 2015 were randomly divided into three groups according to NYHA functional classification: 39 cases of grade 2, 77 cases of grade 3, 65 cases of grade 4 ; The same period, another 120 cases of healthy subjects in our hospital, as a normal control group. BNP levels were detected by ELISA, ALD levels by radioimmunoassay, and cTnI levels by immunoturbidimetry. Results The levels of plasma BNP, ALD and cTnI in DHF group were significantly higher than those in normal control group, cardiac function grade 3 group, cardiac function grade 4 group, cardiac function grade 2 group, and heart function grade 4 group than in cardiac function grade 3 group High, the difference was statistically significant (P <0.01). Correlation analysis showed that there was a significant positive correlation between plasma BNP and ALD and cTn I levels (r = 0.614, P <0.05; r = 0.586, P <0.05). Conclusion The detection of plasma BNP, ALD and cTn I levels is helpful to diagnose DHF, assess its severity, and evaluate its short-term therapeutic efficacy.
