患者女,52岁,因乏力、纳差、怕冷、浮肿进行性加重10年,腹泻半月,昏迷4天而入院。检查:深昏迷,体温31℃,脉搏40次/分,血压80/50。面黄浮肿,未见蜘蛛痣,甲状腺(-),双肺呼吸音粗,心率40次,律齐:心音低,腹水征(±)肝脾不大。白细胞5,493,中性79%,淋巴21%。黄疸指数6单位,TTT 6单位,GPT40单位以内。总蛋白5.5克%,A4.2克%,G1.3克%。血氨121μg%,K~+2.2m Eq/L,Na~+,153.5m Eq/L,C1~-98m Eq
