目的探究与分析右旋糖酐40氯化钠注射液、硫酸庆大霉素注射液、地塞米松磷酸钠注射液、盐酸异丙嗪注射液联合用药对防止妇产科术后盆腹腔粘连的应用。方法采用随机数字法将2003年-2013年间进行妇产科手术的患者分为试验组和对照组,试验组患者在关闭腹腔前留置右旋糖酐40氯化钠注射液100ml、硫酸庆大霉素注射液8万U、地塞米松磷酸钠注射液5mg、盐酸异丙嗪注射液25mg等4种药物的混合液留于腹腔;对照组患者直接进行腹腔缝合。从试验组和对照组患者中随机选取因各种原因需再次进行手术的患者各50例,在再次手术时观察2组患者盆腹腔粘连的情况及第二次手术术后疗效。结果对照组再次手术时发现重度粘连及轻度粘连的发生率均高于试验组,差异有统计学意义(P<0.05)。观察组治疗总有效率为96%高于对照组88%,差异有统计学意义(P<0.05)。结论进行妇产科手术的患者在关闭腹腔前,采用留置右旋糖酐40氯化钠注射液、硫酸庆大霉素注射液、地塞米松磷酸钠注射液、盐酸异丙嗪注射液的混合液,可起到防止盆腔粘连的作用,值得临床推广应用。
Objective To study and analyze the application of dextran 40 sodium chloride injection, gentamycin sulfate injection, dexamethasone sodium phosphate injection and promethazine hydrochloride injection in preventing pelvic adhesions after obstetrics and gynecology. Methods A random number method was used to divide the obstetrics and gynecology patients from 2003 to 2013 into two groups: test group and control group. In the test group, 100 ml of dextran 40 sodium chloride injection and gentamicin sulfate injection 80,000 U, dexamethasone sodium phosphate injection 5mg, promethazine hydrochloride injection 25mg and other 4 kinds of drug mixture was left in the abdominal cavity; the control group patients directly to the abdominal suture. 50 patients in each group were selected randomly from the experimental group and the control group for re-operation due to various reasons. The abdominal adhesions of the two groups and the second postoperative effect were observed in the reoperation. Results In the control group, the incidence of severe adhesions and mild adhesions was higher than that of the experimental group at the time of reoperation, the difference was statistically significant (P <0.05). The total effective rate of observation group was 96% higher than 88% of control group, the difference was statistically significant (P <0.05). Conclusions Obstetrics and gynecology patients with closed intraperitoneal before indwelling dextran 40 sodium chloride injection, gentamicin sulfate injection, dexamethasone sodium phosphate injection, promethazine hydrochloride mixture of injection can be Play to prevent the role of pelvic adhesions, it is worth clinical application.
