目的探讨后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿的手术方法和临床效果。方法回顾性分析18例(21侧)乳糜尿患者行后腹腔镜肾蒂淋巴管结扎术治疗的临床资料。结果18例患者均手术成功,手术时间为单侧(75.3±20.5)min,术中出血量(45.2±17.5)ml,无意外损伤,术后住院(5.5±1.5)d。患者出院时尿液均清亮,18例患者尿乙醚试验全部阴性。随访5～24个月,无复发。结论后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿疗效较好,具有术中出血量少、创伤小、淋巴管结扎彻底术后住院时间短、恢复快等优点,是目前治疗乳糜尿较理想的手术方式。
Objective To investigate the surgical and clinical effects of retroperitoneal laparoscopic lymphatic drainage for chyluria. Methods The clinical data of 18 cases (21 sides) of chyluria treated by retroperitoneal laparoscopic renal pedicle lymphatic drainage were retrospectively analyzed. Results All the 18 patients were operated successfully. The operation time was unilateral (75.3 ± 20.5) min, the amount of blood loss was (45.2 ± 17.5) ml, no accidental injury and the postoperative hospital stay was (5.5 ± 1.5) d. Urine was cleared when discharged from the hospital, and all 18 patients had negative urine ether test. Follow-up of 5 to 24 months, no recurrence. Conclusions Laparoscopic renal pedicle lymphatic ligation is effective in treating chyluria. It has the advantages of less intraoperative blood loss, less trauma and complete lymphatic ligation, shorter hospital stay and faster recovery, which is ideal for the treatment of chyluria.
