目的 :探讨成人病理嗓音电声门图 (EGG)曲线的临床特点。方法 :对 4 5 3例病理嗓音进行EGG检测 ,分析异常EGG曲线的临床意义。结果 :病理嗓音EGG曲线表现 :①EGG周期由接触期 (CP)和开放期 (OP)组成 ,二者比值界于一定范围 ,CP/OP <0 .70或 >2 .95均为异常 ;②CP又分为接触渐闭相 (CCP)和接触渐离相(COP) ;OP又分为渐开开放相 (OOP)和开放渐触相 (OCP) ;③振动周期中各时相形态异常表现 :CCP平缓、CCP切迹、波峰平坦、COP陡直、COP切迹、COP隆起、OOP隆起、OCP隆起 ;④联波、不规则等情况虽然少见 ,但反映了病理嗓音的特殊振动模式 ;⑤总的各时相异常变化中 ,COP >OP >CCP ,各病理嗓音组呈现不同的特点 ;⑥部分病理嗓音表现为联合异常 ,以COP与OOP联合病变为最多 ,单或双侧声带息肉、Reinke水肿及声门型喉癌组均有相当比例的联合病变。结论 :成人病理嗓音EGG曲线的形态变异错综复杂 ,各种变异形式又有一定的内在联系。确立OP的异常形式增加了病理EGG检出率 ,并且与CP异常密不可分 ;不同病理嗓音EGG曲线异常呈现不同倾向性 ,有一定规律可循。确立病理嗓音EGG曲线各时相形态异常变化形式 ,综合分析病理声带的异常振动模式 ,更深入发掘EGG检测的潜力并且完善病理声带振动机制的研究
Objective: To investigate the clinical features of adult pathological voice electro-glottic (EGG) curves. Methods: 455 cases of pathological voice EGG detection, analysis of abnormal EGG curve clinical significance. Results: The EGG curve of pathological voice showed: (1) The EGG cycle consisted of the contact period (CP) and the open period (OP), the ratio of the two was within a certain range, CP / OP <0.70 or> 2.95 were abnormal; (CCP) and contact asymptotic facies (COP); OP is divided into OOP and OCP; ③The morphology of each phase in the vibration cycle is abnormal: CCP Flattening, CCP notch, flat wave crest, steep COP, clear cut of COP, uplift of COP, uplift of OOP, uplift of OCP; ④ rare occasional waves and irregularities, but reflecting the special modes of vibration of pathological voice; COP> OP> CCP, the pathological voice group showed different characteristics; ⑥ part of the pathological voice showed a combination of abnormalities, COP and OOP joint disease is the most, single or bilateral vocal cord polyps, Reinke edema and Glottic cancer group have a significant proportion of joint disease. Conclusion: The morphological variation of EGG curve in adult pathological voice is complex and there are some internal relations among various variants. The establishment of an abnormal form of OP increased the detection rate of pathological EGG and was closely related to the abnormality of CP. The EGG curves of different pathological voice showed different tendencies, with certain rules to follow. To establish the abnormal morphological changes of pathological vocal EGG curves at each phase, to analyze the abnormal vibration patterns of pathological vocal cords, to further explore the potential of EGG detection and to improve the vibration mechanism of pathological vocal folds
