目的探讨有创机械通气用于重症急性心原性肺水肿的疗效及临床分析。方法 2010年6月至2012年6月期间诊治的20例重症急性心原性肺水肿患者,应用有创机械通气进行相应治疗。对患者发绀、肺部湿罗音的变化情况和死亡情况;机械通气前后呼吸、心率和血压的变化;动脉氧分压(PaO2)、二氧化碳分压(PaCO2)和动脉血氧饱和度(SaO2)的变化情况,进行观察和比较。结果经有创机械通气治疗2h后,18例患者的呼吸困难症状均得到明显改善,发绀、肺部湿罗音均消失,2例死亡。与治疗前相比,治疗后心率、呼吸、收缩压和舒张压都明显降低,PaO2、SaO2明显升高,而PaCO2明显降低。结论对于重症急性心原性肺水肿患者采用有创机械通气治疗疗效显著,能够快速缓解症状,明显改善血气指标。
Objective To investigate the efficacy and clinical analysis of invasive mechanical ventilation for severe acute pulmonary heart disease. Methods Twenty patients with severe acute cardiogenic pulmonary edema diagnosed and treated between June 2010 and June 2012 were treated with invasive mechanical ventilation. The change of lung wet rales and the death of the patients; the change of respiration, heart rate and blood pressure before and after mechanical ventilation; the PaO2, PaCO2 and SaO2, The changes, for observation and comparison. Results After 2 hours of invasive mechanical ventilation, the symptoms of dyspnea in 18 patients were significantly improved. Cyanosis and pulmonary wet rales disappeared and 2 patients died. Compared with before treatment, after treatment, heart rate, respiration, systolic and diastolic blood pressure were significantly decreased, PaO2, SaO2 increased significantly, while PaCO2 decreased significantly. Conclusions The treatment of invasive mechanical ventilation for patients with severe acute cardiogenic pulmonary edema has significant curative effect, which can quickly relieve the symptoms and improve the blood gas index obviously.
