目的探讨应用六西格玛方法提高神经内科出院病人2周专科门诊复诊率。方法应用六西格玛质量管理体系中的定义、测量、分析、改进和控制5个步骤对神经内科病人出院至复诊的流程进行分析,找出影响病人复诊的主要原因,制定相应的改进措施,如增加出院常规宣教项目、发放复诊联系卡、与病人或家属协商确定回访时间及方式等,以提高出院病人的复诊率。结果神经内科病人复诊率从29.21%提高到41.67%,差异有统计学意义(χ2=10.448,P=0.001)。结论应用六西格玛方法,通过团队合作实施流程改进,是提高出院病人复诊率的有效办法。
Objective To investigate the application of six sigma method to improve the rate of two-week specialist outpatient visits in patients discharged from neurology department. Methods The definition, measurement, analysis, improvement and control of five steps in Six Sigma quality management system were used to analyze the process of discharge and reexamination of neurological patients. The main reasons that affected patients’ referral were identified and corresponding improvements were made, such as increased discharge Regular mission programs, the issuance of referral contact card, consultation with patients or their families to determine the time and manner of visits, etc., in order to improve the referral rate of discharged patients. Results The rate of referral to neurology patients increased from 29.21% to 41.67%, the difference was statistically significant (χ2 = 10.448, P = 0.001). Conclusion Applying the Six Sigma method to improve the process of discharge through teamwork is an effective way to improve the referral rate of discharged patients.
