目的探讨超声引导经皮穿刺微波消融联合中药治疗子宫肌瘤的临床疗效。方法采用微波消融仪对22例患者24个子宫肌瘤病灶,超声引导经皮穿刺微波消融术后联合中药进行治疗,治疗后1d用增强MRI检测肌瘤血供情况。对有块状排出物者,收集标本送病理检查。结果 22例患者24个子宫肌瘤病灶治疗3个月肌瘤明显缩小,肌瘤内出现无回声区,6～7个月后无回声区逐渐消失。消融后1～8个月内,10例患者(肌壁间肌瘤6个,黏膜下肌瘤4个)月经期有肉样组织排出,排出物病理检查证实为坏死组织。浆膜下肌瘤患者均无坏死组织排出。结论超声引导经皮微波消融联合中药治疗子宫肌瘤是一种安全微创,疗效肯定的治疗方法 。
Objective To investigate the clinical effects of ultrasound guided percutaneous microwave ablation combined with traditional Chinese medicine in the treatment of uterine fibroids. Methods Twenty-four patients with uterine leiomyoma of 22 cases were treated with microwave ablation instrument. Ultrasound-guided percutaneous microwave ablation combined with traditional Chinese medicine was used. After 1 d of treatment, enhanced MRI was used to detect the blood supply of fibroids. For those with massive exudates, collect specimens for pathological examination. Results Twenty-four patients with uterine leiomyoma of fibroids had conspicuously constricted fibroids in 3 months. There was no anechoic area in fibroids, and disappeared after 6 to 7 months. Within 1 to 8 months after ablation, 10 patients (6 intramural fibroids and 4 submucosal myomas) were excreted in the period of menstruation and were confirmed as necrotic tissue by excretion biopsy. Patients with subserosal fibroids no necrosis tissue excretion. Conclusion Ultrasound-guided percutaneous microwave ablation combined with traditional Chinese medicine treatment of uterine fibroids is a safe minimally invasive, effective treatment of certain.
