目的探讨病理性黄疸心肌损伤的发生率及其左心收缩功能的变化。方法对90例新生儿病理性黄疸进行外周血心肌肌钙蛋白I(cTnI)含量测定,同时测定左心收缩功能:射血分数(LVEF)、短轴缩短率(LVFS)、心搏量(SV)、主动脉峰值流速(PFVA),并分析cTnI与左心收缩功能的关系。以52例新生儿生理性黄疸作为正常对照组(简称对照组)。结果高胆组患儿cT-nI测定值显著高于正常对照组(P<0.01),重度黄疸患儿左心收缩功能显著低于正常对照组(P<0.01),cTnI与LVEF、LVFS、SV、PFVA呈直线负相关。结论对新生儿病理性黄疸进行外周血cTnI含量及左心收缩功能检测能早期诊断心肌损伤,了解心功能情况,从而可以早期治疗,改善预后。
Objective To investigate the incidence of pathological jaundice myocardial injury and left ventricular systolic function changes. Methods Ninety newborns with pathological jaundice were involved in the determination of cardiac troponin I (cTnI) levels. Left ventricular systolic function (LVEF), short axis shortening (LVFS), stroke volume (SV) ), Aortic peak flow rate (PFVA), and analysis of cTnI and left ventricular systolic function. 52 cases of neonatal physiological jaundice as a normal control group (referred to as the control group). Results The measured value of cT-nI in children with hypercholesterolemia was significantly higher than that in normal controls (P <0.01). Left ventricular systolic function in children with severe jaundice was significantly lower than that in normal controls (P <0.01) , PFVA showed a linear negative correlation. Conclusion The detection of cTnI in peripheral blood and left ventricular systolic function in neonatal pathological jaundice can early diagnose myocardial injury and understand the cardiac function so that it can be treated early and prognosis can be improved.
