目的介绍带血管蒂骨骺瓣移植的基础研究及相关临床应用。方法回顾总结笔者近20年来在带血管蒂骨骺瓣移植方面所做的相关应用解剖、动物实验及临床应用研究,探讨其临床应用疗效。结果膝下外侧动脉腓骨头骺瓣和臀上动脉深上支髂骨骺瓣均可作为骨骺游离移植的供区。动物实验表明,带血供的骨骺瓣移植能够成活并保持生长能力。1例膝下外侧动脉腓骨头骺瓣移植重建尺骨远端患者,18年随访示修复的尺骨与对侧几近等长且功能良好。结论带血管蒂骨骺瓣移植是修复儿童长骨骨端缺损的有效方法。
Objective To introduce the basic research and related clinical application of vascular pedicle epiphysis. Methods The author summarized the application anatomy, animal experiments and clinical application made by the author in the past 20 years in the transplantation of vascularized epiphysial flap, and explored its curative effect. Results Lower extremity articular fibular epiphysis flap and superior superior iliac ilium epiphysis flap can be used as epiphysis free graft donor area. Animal experiments show that epiphysis with blood supply for graft survival and maintain growth capacity. One case of the distal lateral ulnar artery pedicled with fibular epiphysis flap was reconstructed. The 18-year follow-up showed that the restored ulnar bone was nearly as long as the contralateral side and well-functioned. Conclusion Transvascular pedicle epiphysis flap is an effective method to repair the defect of bone long side of children.
