目的:通过事前干预不合理处方,提高门诊合理用药水平。方法:建立药学服务友情提示卡,就药师审方时发现的不合理内容与医师进行处方调配前沟通;并对我院门诊2007年5月~2009年5月不合理处方进行干预。结果与结论:不合理处方主要表现在剂量、用法不正确,选用剂型与给药途径不合理,未注明过敏试验及结果的判定等。应用药学服务友情提示卡对不合理处方进行干预,使干预门诊处方存在问题的关口前移,患者用药的安全性显著提高。
Objective: To improve the rational use of medicine in outpatient departments by interfering with unreasonable prescription in advance. Methods: The pharmacy service reminder card was set up to communicate with the physician before the prescription was made on the unreasonable content found by the pharmacist in the trial; and the intervention in our clinic from May 2007 to May 2009 was unreasonable. RESULTS AND CONCLUSION: Unreasonable prescriptions mainly manifested in dose, incorrect usage, unreasonable choice of dosage forms and route of administration, failure to indicate allergy test and result determination. The application of pharmacy service tips card to intervene in unreasonable prescriptions, the interference of prescribing prescriptions in front of the problem, the safety of patients with medication significantly improved.
