目的了解潍坊地区喘息性疾病呼吸道病原感染情况以及与喘息性疾病的关系。方法采集2012年1月~2013年3月在潍坊市妇幼保健院儿科住院的喘息性疾病400例患儿的痰标本,用直接免疫荧光法检测呼吸道7种常见病毒—呼吸道合胞病毒(RSV),腺病毒(ADV),流感病毒A(IVA),流感病毒B(IVB),副流感病毒-Ⅰ、Ⅱ、Ⅲ(PIV-Ⅰ、Ⅱ、Ⅲ),细菌培养检测细菌。采集静脉血3ml离心制备血清,采用金标法检测肺炎支原体抗体。结果 400例患儿共检出呼吸道病毒202例,阳性率50.5%,检出肺炎支原体128例,阳性率32%。检出细菌101例,阳性率25.2%。病毒病原中,以呼吸道合胞病毒最常见,细菌病原中以金黄色葡萄球菌最多见。病原混合感染率为28.5%。不同疾病主要感染病原各异,其中毛细支气管炎以病毒感染为主,支气管哮喘以肺炎支原体感染为主,喘息性支气管炎以细菌感染为主。结论本组资料显示呼吸道病毒是潍坊地区喘息性疾病住院患儿的第一病原,肺炎支原体次之,细菌是混合感染的的常见病原,以病毒合并细菌感染为主,其次是病毒合并支原体感染,多见于毛细支气管炎及支气管哮喘组,提示混合感染是毛细支气管炎、支气管哮喘不可忽视的因素,因此进行病原联合检测,明确多种病原是非常重要的。
Objective To understand the prevalence of respiratory tract infection of wheezing diseases and the relationship with asthmatic diseases in Weifang. Methods A total of 400 sputum samples of asthmatic children hospitalized in pediatric department of Weifang Maternal and Child Health Care Hospital from January 2012 to March 2013 were collected. The sputum specimens of 7 respiratory viruses, respiratory syncytial virus (RSV) , Adenovirus (ADV), influenza virus A (IVA), influenza virus B (IVB) and parainfluenza virus I, II and III (PIV-I, II and III). Blood samples were collected by centrifugation 3ml venous blood, Mycoplasma pneumoniae antibody was detected by gold standard method. Results A total of 202 cases of respiratory viruses were detected in 400 children, the positive rate was 50.5%. 128 cases of Mycoplasma pneumoniae were detected, the positive rate was 32%. Bacteria were detected in 101 cases, the positive rate of 25.2%. Of the virus pathogen, the most common respiratory syncytial virus, the most common bacterial pathogen Staphylococcus aureus. The prevalence of mixed infection was 28.5%. The main infectious diseases of different diseases vary, including bronchiolitis virus infection, bronchial asthma Mycoplasma pneumoniae infection, asthmatic bronchitis mainly bacterial infections. Conclusions This group of data shows that respiratory virus is the first pathogen in hospitalized children with wheezing diseases in Weifang region, followed by Mycoplasma pneumoniae. Bacteria are the common pathogens of mixed infection. The main bacterial infection is bacterial infection, followed by the virus combined with mycoplasma infection, More common in bronchiolitis and bronchial asthma group, suggesting that mixed infection is capillary bronchitis, bronchial asthma can not be ignored, so pathogen detection, clear a variety of pathogens is very important.
