清燥救肺汤对胸部肿瘤患者放射治疗3年后肺功能影响临床观察

目的:观察胸部肿瘤患者进行放射治疗时加用清燥救肺汤对患者肺功能的保护作用。方法:将112例胸部肿瘤放射治疗患者随机分为2组各56例,2组患者均采用方案、总剂量及分次剂量均一致的放射治疗,治疗组同时加用清燥救肺汤口服,连续治疗6月;对照组放疗开始至6月不予相关药物干预。观察2组患者1年内放射性肺损伤的发生情况,36月期间肺部影像学改变情况、肺弥散功能(DLCO)下降情况及呼吸困难出现的例数和程度。结果:放射治疗后12月时治疗组和对照组放射性肺损伤发生率分别为26.8%、53.6%,2组比较,差异有非常显著性意义(P<0.01)。36月时肺部损伤面积(ALI)治疗组为97 cm2,对照组为198cm2,2组比较,差异有非常显著性意义(P<0.01)。36月时DLCO下降幅度>25%的患者治疗组为19.6%,对照组为42.9%,2组比较,差异有非常显著性意义(P<0.05)。36月时发生中重度呼吸困难者治疗组6例,对照组16例,2组比较,差异有非常显著性意义(P<0.01)。结论:清燥救肺汤不仅能降低放射性肺损伤的发生率,而且对受照患者的肺功能具有显著保护作用。 Objective: To observe the protective effect of Qingzaojiu Decoction on lung function in patients with chest cancer treated with radiotherapy. Methods: One hundred and twenty-two cases of thoracic tumor radiotherapy were randomly divided into two groups (n = 56). The two groups were treated by radiation therapy with the same dosage, the same dose and the same dosage. The treatment group was treated with Qingzao Yifei Decoction orally, Continuous treatment of June; control group began to radiotherapy in June without relevant drug intervention. The incidence of radiation-induced lung injury within one year, the changes of lung imaging in 36 months, the decline of pulmonary diffusing function (DLCO) and the number and degree of respiratory distress were observed. Results: The incidence of radiation-induced lung injury in the treatment group and control group was 26.8% and 53.6% respectively at 12 months after radiotherapy. There was significant difference between the two groups (P <0.01). The lung injury area (ALI) at 36 months was 97 cm2 in the treatment group and 198 cm2 in the control group. There was a significant difference between the two groups (P <0.01). In 36 months, the rate of decline of DLCO> 19% was 19.6% in the treatment group and 42.9% in the control group. There was significant difference between the two groups (P <0.05). There were 6 cases in the treatment group with moderate-severe dyspnea at 36 months and 16 cases in the control group. There was significant difference between the two groups (P <0.01). Conclusion: Qingzaojiu Tang can not only reduce the incidence of radiation-induced lung injury, but also have significant protective effects on the lung function of irradiated patients.
